Stop Your Panic Attacks
Panic attacks
Also called Anxiety attacks, Panic disorder, Sudden fear
Who it's forA man whose heart races out of nowhere, who thinks he is dying, and who now plans his life around the next attack.
Tonight
- Chest pain that spreads to your arm, neck, jaw or back, or with sweating and breathlessness: call 911 now
- Take the PDSS and write the score down
- Count today’s caffeine, pre-workout included, and cut it tomorrow
The plan
34 moves, in order, after 1 check. Tap any row to see how.
Check first
The number to measure: the PDSSThe number to track: seven questions, every two weeksCheck
The number to measure: the PDSS
For: Knowing where you stand
A 7-question score for how much panic has hold of your life, and whether it is easing.
How to check
- Answer the 7 questions about the past week.
- Write the score down. It runs from 0 to 28.
- Repeat every 2 weeks, and bring the scores to the doctor.
- What it is
- 7 questions
- When
- Every 2 weeks
- How often
- For as long as you run the page
Reading the number
5or less: remission
The scale tracked change with treatment. A score of 5 or less counts as remission. A 40% drop counts as a response.
¹ The American journal of psychiatry, 1997. The study is under The science, below.
The science
- In detail
- Seven questions on how often the attacks come, how bad they are, the dread between them, what you avoid, and what it takes from your work and life. Each is scored 0 to 4. Take it today, then every two weeks.
- Found
- Built as a seven-item scale, it showed excellent agreement between raters and tracked change with treatment. The self-rated form held up too. A score of 5 or less is the proposed line for remission, and a 40% drop for response.
- Why
- Panic swings week to week, and one bad day feels like the whole month. A score on paper every two weeks shows the real trend, and it is the number a doctor tracks.
- How it is done
- Seven questions; Every two weeks; Today, then every 2 weeks
The studies
The Panic Disorder Severity Scale was associated with excellent interrater reliability, moderate internal consistency, and favorable levels of validity and sensitivity to change.

1Know what a panic attack isLearn the attack: it peaks, it passes, it does not harm youModerate evidence
Know what a panic attack is
For: After the first attack
Learn what a panic attack is. It peaks, it passes, and it cannot hurt you.
What to do
- Read what an attack is: the rush, the peak, the pass.
- Note that most last 5 to 20 minutes. Night attacks that wake you are the same wave.
- Chest pain never checked: see a doctor first. Spreading chest pain: 911.
- How much
- One read
- When
- This week
- How long
- Before the next attack
Standard practice
A panic attack is a rush of intense mental and physical symptoms that most often lasts 5 to 20 minutes. It is frightening but not dangerous. Anyone seen in an emergency with an attack has the minimum checks to exclude a physical cause.
What happened in the studies
Eased the first weeks
In a trial, a plain booklet at the start of treatment eased anxiety and panic in the first weeks.
¹ Depression and anxiety, 2002. The study is under The science, below.
Watch out
Chest pain that spreads to your arm, neck, jaw or back: call 911 first.
The science
- In detail
- Learn the attack before the next one. A rush of racing heart, tight chest, breathlessness, shaking, dizziness and the fear you are dying. It peaks and passes, most often within 5 to 20 minutes, and it does not harm you. Attacks can also wake you from sleep: that is the same wave, not a sign of danger. Chest pain you have not had checked goes to a doctor first.
- Found
- In a trial, a plain information booklet given at the start of treatment eased anxiety and panic more than treatment alone over the first weeks. Waking from sleep in panic is common with the disorder.
- Why
- The attack feeds on the belief that the body is failing: heart attack, stroke, going mad. Knowing the shape of it breaks the loop where fear of the symptoms makes the symptoms stronger.
- Watch
- First attack with chest pain, or chest pain that spreads to the arm, neck, jaw or back: call 911 first. Never assume it is only panic.
The studies
The administration of a psychoeducational brochure (SIB) to PD patients at the initiation of therapy had beneficial effects during the first weeks of treatment.

2Slow breathing in an attackStay put, breathe slow, let it passStandard practice
Slow breathing in an attack
For: In the middle of one
Stay where you are and breathe slowly. The attack peaks and passes on its own.
What to do
- Stay where you are.
- Breathe in gently, low into the belly, and let it flow out slowly. Count 1 to 5 each way if it helps.
- No paper bag, no big gulping breaths.
- Tell yourself: this will pass, it is not dangerous.
- How much
- Slow breaths
- When
- The moment it starts
- How long
- Every attack
Standard practice
During an attack: stay where you are, breathe slowly and remind yourself it will pass. Over-breathing is rapid, deep breathing. Let the breath flow into the belly without forcing it, and breathe out slowly. Counting 1 to 5 in and out helps some.
What happened in the studies
Stay put, breathe slow
The guidance for the moment it starts: stay where you are, breathe slowly, and remind yourself it will pass.
¹ Annals of emergency medicine, 1989. The study is under The science, below.
Watch out
Never breathe into a paper bag: it can drop your oxygen. If you feel dizzy or tingly, breathe more gently.
The science
- In detail
- When an attack starts: stay where you are, breathe slowly, and tell yourself it will pass. Breathe in gently, low into the belly, and let it flow out slowly. Counting 1 to 5 each way helps some. Skip the paper bag, and skip big gulping breaths: over-breathing is fast, deep breathing.
- Found
- The guidance for the moment it starts: stay where you are, breathe slowly, and remind yourself the attack will pass and is not life threatening.
- Why
- Fast, shallow breathing blows off carbon dioxide and brings the tingling, dizziness and air hunger that feel like dying. A slow out-breath steadies it, and staying put teaches the brain the attack passes on its own.
- Watch
- Paper-bag rebreathing has killed when a heart or lung problem was mistaken for panic: never use it. Dizziness or tingling means you are blowing off too much carbon dioxide. Breathe lighter, not deeper.
The study
The author reports three cases in which this treatment, erroneously applied to patients who were hypoxemic or had myocardial ischemia, resulted in death.
PMID 2499228 · Hypoxic hazards of traditional paper bag rebreathing in hyperventilating patients. · Annals of emergency medicine, 1989

3CaffeineCount it, then cut it well under 400 mg a dayStrong evidence
Cut the caffeine
For: Coffee or pre-workout daily
Caffeine sets off the racing heart that panic starts from. Count it, then cut it.
What to do
- Count every coffee, energy drink, cola, tea and pre-workout for a week.
- Cut to well under 400 mg a day, none after midday.
- Step down over 1 to 2 weeks.
- How much
- Well under 400 mg
- When
- None after midday
- How long
- Taper over 1 to 2 weeks
Standard practice
Caffeine, alcohol, sugary food and drinks, and smoking can all make attacks worse. For most adults, 400 mg of caffeine a day is the level not generally linked to harm.
What happened in the studies
Attacks in 50% above 400 mg
Above 400 mg, about 4 cups of coffee, caffeine set off panic attacks in half of those tested.
¹ Journal of psychopharmacology (Oxford, England), 2025. The study is under The science, below.
Watch out
Cutting fast brings a headache for a few days. Step it down.
The science
- In detail
- Count the coffee, energy drinks, cola, tea and pre-workout for a week. Then cut well under 400 mg a day, and none after midday. If attacks started with a stimulant, stop it. Step down over a week or two.
- Found
- Above 400 mg, about four cups of coffee, caffeine set off panic attacks in half of those with the disorder in a challenge test. Across 14 studies, caffeine raised anxiety.
- Why
- Caffeine pushes adrenaline up and brings the racing heart, the shake and the jitter. Those are the exact body signals a panic attack starts from, so caffeine loads the trigger.
- Watch
- Cutting fast from a high intake brings headache and tiredness for a few days. Taper it.
The studies
Caffeine in doses above 400 mg, approximately four cups of coffee, induces panic attacks in 50% of individuals with panic disorder (PD) and elevates anxiety

4Drink less, and never to calm downCount the drinks, then cut a levelModerate evidence
Drink less, and never to calm down
For: Drinking to settle
A drink calms you for an hour and brings the racing heart back the next day. Cut back.
What to do
- Count your drinks for a week, and mark the ones for nerves.
- Cut back a level.
- Shake without a drink? See a doctor before you stop.
- How much
- One level less
- When
- Every week
- How long
- A year
Standard practice
Caffeine and alcohol can make attacks worse. Stopping heavy drinking suddenly brings withdrawal: anxiety, a racing heartbeat, sweating and shaking. Get medical help before you stop.
What happened in the studies
Better mental health
In 1,383 people followed for a year, every step down in drinking came with better mental health.
¹ Alcoholism, clinical and experimental research, 2017. The study is under The science, below.
Watch out
If you shake without a drink, stopping suddenly is dangerous. See a doctor first.
The science
- In detail
- Count the drinks for a week. Mark the ones you had to settle your nerves, and the mornings after. Then cut a level. If you drink every day and shake without it, see a doctor before you stop. For more, run Drinking less.
- Found
- Better mental health, one year. 1,383 people followed for a year: every step down the risk-drinking ladder came with fewer alcohol problems and better mental health. The bigger the cut, the bigger the gain.
- Why
- A drink settles the nerves for an hour, then the rebound comes: a racing heart, sweats and dread the next day, the same signals an attack runs on. Cutting the count breaks that loop.
- Watch
- If you drink every day and get shakes, sweats or a racing heart without it, stopping suddenly is a medical danger. Get medical help before you stop.
The study
Any reduction in […] risk drinking level during treatment was associated with significantly fewer alcohol-related consequences and improved mental health at the end of treatment and for up to 1 year posttreatment.

5Stop the cannabisStop the weed while you fix the panicModerate evidence
Stop the cannabis
For: Smoke or vape weed
Cannabis can set off attacks and keep them coming. Stop while you fix the panic.
What to do
- Stop, or cut hard, starting this week.
- Note whether the attacks ease over the next weeks.
- Struggling to stop? Run the Quit weed page too.
- How much
- Stop, or cut hard
- When
- This week
- How long
- For the length of the page
Standard practice
Using cannabis can make you anxious, panicky, or even aggressive, and can cause hours of anxiety and paranoia.
What happened in the studies
Higher risk of attacks
In large surveys, cannabis use and dependence came with more panic attacks and more panic disorder.
¹ Journal of psychiatric research, 2006. The study is under The science, below.
Watch out
The first weeks off can bring poor sleep and nerves. It passes.
The science
- In detail
- If you smoke or vape weed, stop, or cut hard while you work this page. Attacks that started with cannabis, or come on while high, are a reason to stop now. For the how, run Quit weed.
- Found
- In a large household survey, cannabis dependence came with a higher risk of panic attacks. Followed from the teens into the twenties, cannabis use and dependence came with higher odds of panic attacks and panic disorder.
- Why
- THC can set off a racing heart, a sense of unreality and sudden dread, which the brain can learn as an attack. Daily use also leaves a jumpy rebound between highs.
- Watch
- Stopping daily use can bring a few weeks of irritability, poor sleep and anxiety. It passes. Tell the doctor.
The studies
Structured interview data suggest lifetime cannabis dependence is significantly associated with an increased risk of panic attacks.
PMID 16271364 · Lifetime associations between cannabis, use, abuse, and dependence and panic attacks in a representative sample. · Journal of psychiatric research, 2006

6Hard cardio, three times a weekHard cardio, 30 minutes, three times a weekModerate evidence
Hard cardio, three times a week
For: Scared of a racing heart
Hard cardio makes the racing heart on purpose, so the body learns it is safe.
What to do
- Run, row, cycle or swim 3 times a week.
- 30 minutes at a pace where you breathe hard.
- Build up over the first weeks. Keep going through the feelings.
- How much
- 30 minutes, hard
- When
- 3 times a week
- How long
- 8 to 10 weeks
Standard practice
The benefits of exercise as part of good general health should be discussed with everyone who has panic disorder. Regular physical exercise reduces stress and tension.
What happened in the studies
Symptoms down
In a trial, 10 weeks of running cut panic symptoms more than a dummy pill.
¹ The American journal of psychiatry, 1998. The study is under The science, below.
Watch out
The first sessions can bring on the feelings you fear. Keep going. Chest pain when training: see a doctor.
The science
- In detail
- Run, row, cycle or swim three times a week, 30 minutes at a pace that makes you breathe hard. It raises the same heart rate and breathing panic does, on purpose, so the body learns those feelings are safe.
- Found
- Symptoms down against placebo. Ten weeks of regular running cut panic symptoms against placebo on every main measure in a trial. Added to CBT, 30 minutes of hard cardio three times a week kept the gains more than low-intensity exercise.
- Why
- A pounding heart and hard breathing are what a panic attack feels like. Making them on purpose, again and again, teaches the brain they are not a threat, and fitter hearts race less at rest.
- Watch
- Early sessions can bring on the feelings you fear. Build up, and keep going through them. Chest pain on exertion goes to a doctor first.
The studies
In comparison with placebo, both exercise and clomipramine led to a significant decrease in symptoms according to all main efficacy measures (analysis of variance, last-observation-carried-forward method and completer analysis).

Sleep seven to eight hours
For: Everyone
Better sleep lowers anxiety. Fix your sleep and the alarm often quietens with it.
What to do
- Sleep 7 to 8 hours.
- Wake at the same time every day.
- Woken by attacks at night? Tell your doctor; see Stay asleep for the rest.
- How much
- 7 to 8 hours
- When
- Same wake time daily
- How long
- For good
What happened in the studies
Less depression and anxiety
In 65 trials with 8,608 people, better sleep brought depression and anxiety down. The more sleep improved, the more mood did.
¹ Sleep medicine reviews, 2021. The study is under The science, below.
Watch out
Sleeping 9 hours or more and still tired is a question for a doctor.
The science
- In detail
- Seven to eight hours, the same wake time seven days a week. A short night lowers the threshold for the next attack.
- Found
- g = 0.63 on depression. Sixty-five trials, 8,608 people: when a treatment improved sleep, depression fell by a medium effect. Anxiety and brooding fell with it. The more sleep improved, the more mood did.
- Why
- A short night raises the amygdala's reactivity and cuts the prefrontal brake on it the next day. Regular sleep is the one move here that every other move depends on.
- Watch
- Nine hours or more a night that does not refresh is a doctor's question, not a reason to sleep in.
The study
Improving sleep led to a significant medium-sized effect on composite mental health (g+ = -0.53), depression (g+ = -0.63), anxiety (g+ = -0.51), and rumination (g+ = -0.49)

8Breathing training, four weeksFour weeks of daily breathing trainingModerate evidence
Breathing training, four weeks
For: Want a skill ready
Four weeks of daily breathing practice, so the slow breath is there when an attack comes.
What to do
- Practice slow breathing daily for 4 weeks.
- Use a written guide, an app, or a breath-rate device.
- Score the PDSS before and after.
- How much
- Short daily sessions
- When
- Daily
- How long
- 4 weeks
What happened in the studies
Large drop in severity
In a trial, 4 weeks of breathing training cut panic severity a lot, and the gains held at 6 months.
¹ The Journal of clinical psychiatry, 2012. The study is under The science, below.
Watch out
Dizzy or tingly means breathe lighter.
The science
- In detail
- Train the slow breath daily between attacks, so it is ready when one comes. Four weeks of short daily sessions, from a written guide or a breathing app. A device that shows your breath rate or carbon dioxide helps some.
- Found
- Effect sizes 1.34 and 1.53. A trial scored on the PDSS: four weeks of breathing training cut panic severity by large effects, and the gains held at six months. A second study with breath feedback: 83% responded and 54% went into remission.
- Why
- Panic runs on over-breathing and the fear of the sensations it brings. Daily training slows the resting breath and exposes you to the feelings in a safe dose, so they lose their charge.
- Watch
- Dizziness or tingling means you are breathing too hard. Breathe lighter.
The studies
Results Using the Panic Disorder Severity Scale as a primary outcome measure, we found that both breathing training methods effectively reduced the severity of panic disorder 1 month after treatment and that treatment effects were maintained at 6-month follow-up (effect sizes at 1-month follow-up were 1.34 for the raise-CO(2) group and 1.53 for the lower-CO(2) group; P < .01).

What to do
- Sit and breathe about six times a minute: in for 4, out for 6.
- Low into the belly, never forced.
- At least 5 minutes, every day.
- A heart-rate strap or wearable can show the feedback.
- How much
- Six breaths a minute
- When
- Daily
- How long
- At least 5 minutes a day
Standard practice
A calming breathing technique for stress, anxiety and panic. It helps most when done regularly, as part of the daily routine, for at least 5 minutes.
What happened in the studies
Large drop in anxiety
Across 24 studies, slow-breathing training with heart-rate feedback cut stress and anxiety a lot compared with control.
¹ Psychological medicine, 2017. The study is under The science, below.
Watch out
Dizzy or tingly means breathe lighter.
The science
- In detail
- Practice slow breathing every day between attacks, so the skill is there when one comes. About six breaths a minute: in for four, out for six, low into the belly. At least 5 minutes a day. A strap or wearable that shows your heart rate as you breathe gives the feedback the trials used.
- Found
- g = 0.83 against control. Twenty-four studies, 484 people: slow-breathing training with heart-rate feedback cut self-reported stress and anxiety by a large effect against control.
- Why
- At about six breaths a minute the heart rate swings widest with each breath, and every slow out-breath brakes the heart through the vagus nerve. Trained daily, the slow breath becomes the habit the body reaches for when the alarm goes off.
- Watch
- Dizziness or tingling means you are breathing too deep. Breathe lighter, not deeper.
The study
The between-groups analysis comparing biofeedback to a control condition yielded Hedges' g = 0.83.

- Lesson 1: learn what an attack is and what it does.
- Lesson 2: the slow breath, and what your body is doing in an attack.
- Lesson 3: bring on the feelings on purpose, until they stop scaring you.
- Lesson 4: write the scary thought down, like "my heart will stop", and check it against every attack so far.

- How much
- Six lessons
- When
- A lesson every five days
- How long
- 66 days
Start the quest: Stop the Panic
Standard practice
For mild to moderate panic, the first step is self-help, with a check-in with your doctor likely every 4 to 8 weeks.
What happened in the studies
Large drop in panic
In 9 trials, online self-help courses cut panic a lot compared with waiting, and the gains held for months.
¹ Cognitive behaviour therapy, 2020. The study is under The science, below.
Watch out
Chest pain never checked: see a doctor before week 3. Attacks wrecking your life, or dark thoughts: the doctor now.
The science
- In detail
- Start the guided quest. It is built on the methods tested in trials, and you do it on your own. A lesson opens every five days or more. The lessons cover what an attack is, riding it out, and bringing on the feelings on purpose. Then come testing the scary thought, going back into the places you avoid, and the plan for a bad week. See your doctor every 4 to 8 weeks while you do it.
- Found
- g 1.22 against waiting. Nine trials of online self-help courses: panic fell by a large effect against waiting or information alone, and agoraphobia fell too. The gains held at 3 to 6 months, and courses with less doctor support matched those with more. A ten-week self-help book, worked through alone, improved every measure, and the gains held three months on.
- Why
- Panic runs on fear of the feelings and of what they seem to mean. The course takes each piece in turn. It covers what the attack is, the feelings brought on safely, the scary thought checked against the facts, and the places taken back.
- Watch
- Chest pain you have never had checked goes to a doctor before week 3.

In a trial, 12 weeks of supervised sprints used this way cut panic and attacks more than relaxation, and the gains held at 24 weeks.
¹ Frontiers in psychiatry, 2025. The study is under The science, below.
Watch out
Heart or lung trouble, asthma, epilepsy or fainting: ask a doctor first. Stop at any chest pain.
The science
- In detail
- Make the feelings your attacks start with, on purpose, at home, and stay with them until they fade. Pounding heart: sprint, or run hard on the spot. Dizzy: spin in a chair. Short of breath: breathe through a thin straw. Light-headed: breathe fast and hard for a short burst. Rate the fear before and after, and repeat daily until it barely rises. The trials ran it with support: supervised brisk walks with 30-second sprints for 12 weeks, or an online course with a coach. A training partner or a coach at the gym can play that part.
- Found
- Beat relaxation, held 24 weeks. Both trials ran it with support. In a trial, 12 weeks of supervised brisk walking with 30-second sprints, used to bring on the feared feelings, cut panic severity and attacks more than relaxation training, and the gains held at 24 weeks. An online course with a coach’s support that added these exercises cut panic by a large effect against a waiting list, and 43% no longer met the criteria for the disorder.
- Why
- The attack feeds on fear of a pounding heart, dizziness and air hunger. Making them yourself, again and again, with nothing bad happening, teaches the brain they are not a threat, and the attacks lose their fuel.
- Watch
- Check with a doctor first if you have a heart or lung condition, asthma or epilepsy, or have ever fainted. The feelings are meant to be uncomfortable for a few minutes. Stop at any chest pain.
The studies
A 12-week BIE program used as interoceptive exposure was feasible and more effective than relaxation training in reducing panic symptom severity and panic attack frequency, with effects sustained for at least 24 weeks.

12Go back to the places you avoidGo back to what you avoid, in steps, without the propsStrong evidence
Go back to the places you avoid
For: Avoiding places
Go back to the places you avoid, one step at a time, and leave the safety props at home.
What to do
- List what you avoid and rank it, easiest first.
- Go back to the first one and stay until the fear drops.
- Leave the props at home: the exit seat, the pills in the pocket.
- How much
- One step at a time
- When
- Several times a week
- How long
- Until the list is done
Standard practice
Avoiding situations for fear they will trigger an attack creates a cycle of living in fear of fear, which can bring more attacks.
What happened in the studies
Strongest effect on avoidance
Across 34 studies, going back to avoided places had the most consistently strong effects. Dropping safety habits cut anxiety more.
¹ Journal of behavior therapy and experimental psychiatry, 1992. The study is under The science, below.
Watch out
The fear rises before it falls. Stay until it drops.
The science
- In detail
- List the places and things you now avoid: the highway, the train, the gym, the crowd, the coffee. Rank them. Go back to them in steps, easiest first, and stay until the fear drops. Leave the safety habits at home.
- Found
- The strongest effects on avoidance. Pooled across 34 treatment studies, graded exposure to the avoided places had the most consistently strong effects. In an experiment, dropping safety habits during exposure cut catastrophic beliefs and anxiety more than keeping them.
- Why
- Avoiding the place, or going only with the water bottle, the pills in the pocket or the exit in sight, stops the brain learning that nothing bad happens. Going back without them lets the fear be tested and fade.
- Watch
- Expect the fear to rise before it falls. Stay put until it drops, then repeat. Chest pain on exertion goes to a doctor first.
The studies
Exposure had the most consistently strong effect sizes.

The science
- In detail
- Early research. Myo-inositol powder, 12 g a day in the first trial, stirred into a drink and split through the day, for four weeks before you judge it. Beside the rest of the page, never in place of it.
- Found

- Why
- Inositol feeds the second-messenger system that serotonin receptors signal through, so it may work a step downstream of the SSRIs.
- Watch
- Side effects were minimal in the trials. It is a large dose of powder. Tell the doctor if you take it beside a prescription.
The studies
The frequency and severity of panic attacks and the severity of agoraphobia declined significantly more after inositol than after placebo administration.

The science
- In detail
- Early research. A yoga class with postures, breathing and a quiet end, beside the rest of the page. The larger trial ran an hour, five days a week, for twelve weeks.
- Found
- Anxiety down more. A trial of 64 people on usual care: adding yoga cut anxiety more and lifted quality of life across the board. A smaller trial: yoga eased panic symptoms, beliefs and body sensations.
- Why
- Yoga raises the heart rate and the breath and then brings them down, a gentle rehearsal of the rise and fall of an attack, with slow breathing built in.
The studies
Between-group comparisons revealed significant differences in anxiety reduction (p = 0.042) and quality of life enhancement (p < 0.001), favouring the yoga group.

- Found
- g 0.22 on anxiety. Fifteen trials of online mindfulness courses: small gains on anxiety, depression and wellbeing, and a moderate one on stress. More sessions went with bigger gains on stress.
- Why
- Panic runs on fear of the fear. Mindfulness trains you to watch a racing heart as a sensation that rises and passes, not a signal of disaster.
- Watch
- Keep the sittings short and daily. Long silent retreats can stir distress.
The study
Results showed that online MBIs have a small but significant beneficial impact on depression (g=0.29), anxiety (g=0.22), well-being (g=0.23) and mindfulness (g=0.32).

¹ Journal of behavior therapy and experimental psychiatry, 2003. The study is under The science, below.
The science
- In detail
- Early research. Teach yourself to relax the whole body in seconds, in stages, from a self-help course. First tense and let go of each muscle group. Then let go without tensing, then on a cue word, then in a few seconds, anywhere. Practice twice a day, then use it at the first sign of an attack.
- Found
- d 0.71, self-taught. Taught as an online self-help course with brief email contact, applied relaxation brought a medium to large drop in panic, d 0.71. In the first trial of the method, 82% were free of attacks at follow-up.
- Why
- An attack builds from small body signals. Catching the first one and letting the tension go stops the build before it becomes a full wave.
The studies
Overall, the results suggest that Internet-administered self-help plus minimal therapist contact via e-mail has a significant medium to large effect (Cohen's d=0.71 for AR and d=0.42 for CBT).

Sexual side effects. Never stop suddenly.
The science
- In detail
- If you choose a medicine, the UK’s NICE guideline offers an SSRI licensed for panic. Sertraline is labeled for panic in the UK and the US, on a prescription. The label starts at 25 mg a day for a week, then 50 mg. It rises weekly if needed, up to 200 mg.
- Found
- A 12-week trial of 178 people: sertraline cut panic attacks, unexpected ones included, and time spent worrying more than placebo, and it was safe. Across 30 trials pooling all antidepressants, for every seven treated, one more got better than on a dummy pill.
- Why
- SSRIs block the pump that clears serotonin, so more of it stays working on the brain’s alarm circuits. The calm builds over weeks as the brain adapts, which is why it is judged at twelve.
- Watch
- Boxed warning: antidepressants raised suicidal thoughts and behavior at younger ages in short trials, 5 more per 1,000 at ages 18 to 24. Watch closely in the first months and after any dose change. Never with an MAOI, or within 14 days of one, and never with pimozide. Serotonin syndrome with other serotonin drugs, triptans and tramadol among them; tell the doctor about any cocaine use. Bleeding risk with aspirin, NSAIDs or blood thinners. It raises the level of some drugs, warfarin and CYP2D6 drugs among them. Can trigger mania: tell the doctor about any highs. Seizures. Angle-closure glaucoma in narrow-angle eyes. Low blood sodium: headache, confusion, unsteadiness. A heart-rhythm change (QTc) with other risk factors. Anxiety can rise for the first weeks, which is why the dose starts low. Sexual side effects: delayed or failed ejaculation, lower drive, erectile trouble. Never stop suddenly.

An SSRI labeled for panic in the US. Its long half-life makes a missed dose gentler.
What to do
- Ask the doctor if it suits you.
- Start at 10 mg a day for a week, then 20 mg.
- Give dose changes weeks to show.
- How much
- 10 mg, then 20 mg
- When
- Once a day
- How long
- At least 6 months if it works
Standard practice
Prescription only. The US label covers the acute treatment of panic: 10 mg a day, then 20 mg after one week, with trials at 10 to 60 mg. It is not on the UK list of drugs licensed for panic.
What happened in the studies
More panic-free
In a 12-week trial, more were panic-free on fluoxetine than on a dummy pill by week 6 and at the end.
¹ The British journal of psychiatry, 2001. The study is under The science, below.
Watch out
Boxed warning: at 24 and under it can raise suicidal thoughts at first. Never with an MAOI, pimozide or thioridazine. Dangerous with other serotonin drugs. Rash, manic highs, seizures, bleeding, low sodium, a heart-rhythm change. Nerves and poor sleep early on. Can slow reactions. Stopping can bring irritability and dizziness: taper with the doctor. Sexual side effects.
The science
- In detail
- An SSRI labeled for panic in the US, on a prescription; in the UK it is not licensed for panic, so use is off-label. The US label starts at 10 mg a day, then 20 mg after a week. Trials ran 10 to 60 mg.
- Found
- More panic-free than placebo. A 12-week trial: more were panic-free on fluoxetine than on placebo at six weeks and at the end. Twenty mg a day was safe and effective.
- Why
- It blocks the serotonin pump like the others, with a very long half-life. That long tail makes a missed dose or a stop gentler, and makes dose changes slow to show.
- Watch
- Boxed warning: antidepressants raised suicidal thinking and behavior at younger ages. Watch closely early and after dose changes. Never with an MAOI, within 14 days of stopping one or within 5 weeks of stopping fluoxetine, and not started on linezolid or intravenous methylene blue. Never with pimozide or thioridazine. Serotonin syndrome with other serotonin drugs. Stop at a rash or allergic signs. Can trigger mania: screened for bipolar first. Seizures. Weight loss. Bleeding risk with NSAIDs, aspirin or warfarin. Angle-closure glaucoma. Low blood sodium. Anxiety and poor sleep can come early. QT lengthening and rhythm trouble. Can slow judgment and reactions: care with machinery. A long half-life: dose changes take weeks to show. Stopping, most of all suddenly, can bring low mood, irritability, agitation and dizziness: taper with the doctor. Sexual side effects.
The study
Conclusions Fluoxetine at a dose of 20 mg daily is safe and efficacious in reducing symptoms of panic disorder.
PMID 11731354 · Efficacy of usual antidepressant dosing regimens of fluoxetine in panic disorder: randomised, placebo-controlled trial. · The British journal of psychiatry, 2001
The sentence around it
Conclusions Fluoxetine at a dose of 20 mg daily is safe and efficacious in reducing symptoms of panic disorder.
Results Fluoxetine was associated with a statistically significantly greater proportion of panic-free patients compared with placebo after 6 weeks and at end-point.

33Venlafaxine, an SNRIVenlafaxine XR, 37.5 mg then 75 mg, if an SSRI did not suitStrong evidence
Venlafaxine, an SNRI
For: The SSRIs did not suit
A different class of antidepressant, licensed for panic. Blood pressure gets checked.
What to do
- Ask the doctor about an SNRI if an SSRI did not help.
- Start at 37.5 mg a day with food for 7 days, then 75 mg.
- Get your blood pressure checked. Never stop suddenly.
- How much
- 37.5 mg, then 75 mg
- When
- Once a day, with food
- How long
- At least 6 months if it works
Standard practice
Prescription only, licensed for panic in the UK and the US. It starts at 37.5 mg a day for 7 days, then 75 mg, up to 225 mg, with steps at least 7 days apart on the US label and 2 weeks on the UK label. Blood pressure is controlled first and checked through treatment.
What happened in the studies
Up to 61% panic-free
In a 12-week trial of 664 people, up to 61% were panic-free on the active drugs, against 35% on a dummy pill.
¹ Depression and anxiety, 2007. The study is under The science, below.
Watch out
Boxed warning: at 24 and under it can raise suicidal thoughts at first. Never with an MAOI. Raises blood pressure, so it gets checked. Dangerous with other serotonin drugs. Bleeding, low sodium, seizures, manic highs, an eye-pressure emergency. Sexual side effects. More dangerous in an overdose. Strong withdrawal: taper slowly.
The science
- In detail
- If the SSRIs did not suit: venlafaxine extended-release, on a prescription, labeled for panic in the UK and the US. The label starts at 37.5 mg a day for 7 days, then 75 mg, up to 225 mg. Steps are at least 7 days apart in the US, 2 weeks in the UK.
- Found
- Up to 61% against 35% panic-free. A 12-week trial of 664 people: 54% to 61% were panic-free on venlafaxine or paroxetine, against 35% on placebo. About three in four responded, and nearly half went into remission.
- Why
- An SNRI blocks the pumps for both serotonin and norepinephrine. The second chemical adds a push on the brain’s alarm-control circuits that some respond to when an SSRI did not.
- Watch
- Boxed warning: antidepressants raised suicidal thoughts and behavior at younger ages, 5 more per 1,000 at 18 to 24. Watch closely early and after dose changes. Never with an MAOI or within 14 days of one, linezolid or intravenous methylene blue among them. Serotonin syndrome with other serotonin drugs. Raises blood pressure: it is controlled first and checked regularly. Bleeding risk with aspirin, NSAIDs or blood thinners. Angle-closure glaucoma. Can trigger mania: screened for bipolar first. Seizures. Low blood sodium. Rare lung inflammation: report a new cough or breathlessness. Sexual side effects. More dangerous than the SSRIs in an overdose. A strong withdrawal: taper, never stop suddenly.
The study
Panic-free rates at end point with active treatment ranged from 54% to 61%, compared with 35% for placebo.

34Imipramine or clomipramine, after twelve weeksA tricyclic, if an SSRI did not suit after 12 weeksModerate evidence
Imipramine or clomipramine, after twelve weeks
For: SSRIs did not work after 12 weeks
An older antidepressant for panic that did not shift. Off-label, and more side effects.
What to do
- Only after an SSRI did not suit or did not work.
- Start low and raise slowly, as the doctor says.
- Tell the doctor about any heart trouble. Never stop suddenly.
- How much
- Low start, slow rise
- When
- Daily, often at bedtime
- How long
- At least 6 months if it works
Standard practice
If an SSRI is not suitable or there is no improvement after a 12-week course, imipramine or clomipramine may be considered, off-label. Antidepressants start low and rise slowly. The highest overdose risk is with this class.
What happened in the studies
Attacks markedly reduced
In a 12-week trial, both drugs markedly cut panic attacks and the anxiety between them. A dummy pill barely did.
¹ Journal of clinical psychopharmacology, 1992. The study is under The science, below.
Watch out
Boxed warning: at 24 and under it can raise suicidal thoughts at first. Never with an MAOI. Not after a recent heart attack; the heart is watched. Very dangerous in an overdose. Seizures. Dry mouth, constipation, trouble peeing, blurred vision, an eye-pressure emergency. Dangerous with other serotonin drugs. Sexual side effects, erection and ejaculation trouble among them. Worse with alcohol. Stop slowly.
The science
- In detail
- If an SSRI did not suit, or there is no gain after a 12-week course: one of two older tricyclic antidepressants, on a prescription and off-label for panic in the UK and the US. Started low and raised slowly. The clomipramine label starts at 25 mg a day.
- Found
- In a 12-week trial against a placebo, both drugs markedly cut panic attacks and the anxiety between them, and clomipramine did more. In a 10-week trial, clomipramine cut symptoms against placebo on every main measure.
- Why
- Both block the serotonin and norepinephrine pumps, clomipramine leaning on serotonin. They also hit other receptors, which brings the dry mouth, drowsiness and heart effects.
- Watch
- Boxed warning: antidepressants raised suicidal thinking and behavior at younger ages. Watch closely early. Never with an MAOI or within 14 days of one. Clomipramine is not started on linezolid or on methylene blue given into a vein. Ruled out in the recovery weeks after a heart attack. With any heart disease, the heart is watched at every dose for rhythm and conduction trouble. Far more dangerous than the SSRIs in an overdose. It makes seizures more likely. On clomipramine, seizures are the biggest risk. Dry mouth, constipation, trouble passing urine, blurred vision, and angle-closure glaucoma in narrow-angle eyes. Extra care with an overactive thyroid or thyroid pills, and with methylphenidate. It adds to the sedation of alcohol. Low blood sodium. Serotonin syndrome with other serotonin drugs. Sexual side effects: lower or raised drive, erection trouble, and on clomipramine often failed ejaculation. Stop slowly.
The studies
At the end of the trial, the number of panic attacks as well as the anxiety between attacks (measured using the Hamilton Rating Scale for Anxiety) were markedly reduced in patients treated with either of the two antidepressant drugs

How you'll know it's working
- Track
- PDSS score
- How to track
- Seven questions on the attacks, the dread between them and what you avoid. Take it today, then every two weeks. Count the attacks each week in the diary too.
- When to judge
- The self-help course runs six weeks, one lesson a week, with a doctor's check every 4 to 8 weeks. A medicine is reviewed at 2, 4, 6 and 12 weeks, and judged at 12.
Why this happens
Out of nowhere the heart pounds, the chest goes tight, the air will not come, and a voice says you are dying. It can hit in the car, in a meeting, in bed. Then it passes, and you spend the next week waiting for the next one.
Soon you are checking your pulse, skipping the gym, leaving the coffee, avoiding the highway and the crowded train. The fear of the attack starts to run your life more than the attacks do.
The alarm
A panic attack is the body’s danger alarm going off with no danger there. Adrenaline, a pounding heart and fast breathing peak and fall within minutes. It feels like dying, and it does not harm you.
The loop
The attack is not what keeps panic going. The fear of the next one is: checking your pulse, avoiding the feelings, the places, the coffee. Bringing on the feelings on purpose, and going back to the places you avoid, breaks that loop: the feelings stop frightening you.
The chemistry
Caffeine, drink, cannabis and stimulants load the trigger. When the attacks will not shift, the medicines act on serotonin and norepinephrine. They go through a doctor, and they work best beside the rest.
Week by week
Why this order
Rule out the body first, self-help next, then a medicine by preference or if the attacks are heavy, and a specialist if two treatments have not worked.
| When | Add | Why then |
|---|---|---|
| Today | Chest pain that spreads, or comes with sweating and breathlessness: 911 now. Then the PDSS. | The heart comes first, then safety, then the number that says where you start. |
| Week 0 | Learn what an attack is. Slow breathing when one comes. Caffeine, pre-workout, drink and weed counted and cut. Hard cardio three times a week. Book your doctor for the checks. | The first step is to rule out the body and learn the attack. The daily moves start with it. |
| Week 1 | Start the six-week self-help course: lesson one, what an attack is. Four weeks of breathing training and a daily slow-breathing practice beside it. Week 3 of the course: bring on the feelings on purpose. | Self-help is the first step in the UK’s NICE guideline for mild to moderate panic, and you can start it tonight. |
| Week 8, the review | This PDSS against the first. Down 40% or more: keep going, and keep bringing on the feelings and going back to places. Flat, or the attacks are wrecking work and life: the doctor, with the diary, about a medicine. | A 40% drop is the line for a response. Moderate or severe panic that has not moved goes to the doctor. |
| On a medicine | An SSRI licensed for panic, sertraline first. Under 30, a check within a week and weekly for a month. Reviewed at 2, 4, 6 and 12 weeks. If it works, at least 6 months after the best dose. Never stop suddenly. | Nerves can rise in the first weeks, the suicidality warning sits early, and stopping early brings relapse and withdrawal. |
| If the first drug fails | After 12 weeks with no gain: an antidepressant from the other class, venlafaxine, or a tricyclic. | The order the UK’s NICE guideline uses at the 12-week check. |
| If two treatments have not worked | Referral to a specialist mental health service for a full reassessment. | After two treatments with the symptoms still heavy, the UK’s NICE guideline refers you to a specialist. |
| Any week | Constant worry between the attacks: Stop worrying. Nerves before one big event: Stay calm. Wired and unable to wind down: Switch off. | Each rides with panic, and each has its own page. |
What to track
One number: the PDSS score, every two weeks. Five or less is remission, and a 40% drop from the first score is a response. Read the trend over a month, not one score.
The diary is a week: attacks and how bad, caffeine, drinks, weed, days trained, and one line on what you avoided. Where the score has not moved by the review, the order above says what to do next.
The diary, one week

- The PDSS every two weeks: judged at the review, around week 8, then monthly.
- Attacks a week: the number that falls first when the moves work.
- Caffeine, drinks and weed a day: the columns that move fastest.
- On a medicine: mood, sleep, sex and any new dark thoughts, one line a week. A change is a call to the doctor, not a column to watch.
- A heart trace (ECG) if the heart races or skips, and a thyroid blood test if you run hot and shaky. They are checks, not moves.

Words on this page
- Standardized mean difference (SMD)
- A way to put results measured on different scales onto one scale. About 0.2 is a small effect, 0.5 medium and 0.8 large.
- 95% CI
- The range the true result probably sits in. The narrower the range, the surer the result.
- Meta-analysis
- Many studies pooled into one result.
- RCT
- A randomized controlled trial: people put by chance on the treatment or on a dummy version.
- PMID
- A study's ID number in PubMed, the public index of medical research.
- Strong, moderate and early evidence
- Strong: many human trials agree. Moderate: human studies point the same way. Early: the first small human studies.
- PDSS
- Panic Disorder Severity Scale: seven questions on the attacks, the dread and the avoidance. Five or less is remission.
- Panic disorder
- Repeated, unexpected attacks, then at least a month of worry about the next one.
- Interoceptive exposure
- Bringing on the feared body feelings on purpose, like a pounding heart or dizziness, until they stop frightening you.
- CBT
- Cognitive behavioral therapy: the method tested in the trials behind the self-help course. It tests the thoughts and habits that keep panic going.
- SSRI
- Selective serotonin reuptake inhibitor: the first-choice drug class, sertraline and paroxetine among them.
- SNRI
- An antidepressant that acts on serotonin and norepinephrine, venlafaxine among them.
- Tricyclic
- An older antidepressant class, imipramine and clomipramine among them. More side effects, and dangerous in an overdose.
- ECG
- A heart trace: pads on the chest record the heart’s electrical rhythm.
- Boxed warning
- The strongest warning on a US drug label, printed in a box at the top.
- Benzodiazepine
- A fast sedative class, diazepam and alprazolam among them. The label warns of addiction, dependence, and withdrawal seizures if stopped suddenly.
The PDSS tonight, the caffeine counted, and the next attack met with a slow breath instead of a run. The self-help course and bringing on the feelings on purpose do the heavy lifting, and a medicine comes next if the score has not moved. Everything else is added one at a time and judged on the score.
When to see a doctor
- Call 911 now for chest pain or discomfort that does not go away. The same if it spreads to your arm, neck, jaw, stomach or back. The same if it comes with sweating, feeling nauseous, light-headed or short of breath. When in doubt, call.
- A racing or pounding heart that does not settle, or with chest pain, breathlessness or fainting: call 911 or go to the ER.
- New or worse anxiety, agitation or dark thoughts after starting an antidepressant or changing the dose: call the doctor right away.
- A first attack with chest pain, or chest pain that comes and goes: see a doctor to rule out the heart before you call it panic.
- Palpitations that keep coming back, last more than a few minutes, or a family history of heart problems: see your doctor and ask for a heart trace (ECG). A heart rhythm problem can feel like panic.
- A racing heart, shaking, feeling hot, weight loss and nerves together: ask for a thyroid blood test. An overactive thyroid can mimic panic.
- Attacks that started with coffee, energy drinks, pre-workout, cocaine or cannabis: cut them, then tell the doctor what you took. Each can set off a racing heart and panic.
- On a stimulant medicine, ADHD drugs such as methylphenidate among them: new anxiety or a racing heart is a reason to call the doctor.
- Attacks that began when you cut down heavy drinking, or came off anabolic steroids: tell the doctor. Alcohol withdrawal is a medical danger, so get help before you stop.
- Waking at night gasping or choking, with loud snoring and tiredness all day: ask about sleep apnea before you call it night panic. See Sleep apnea.
- Benzodiazepines, such as diazepam or alprazolam, carry a boxed warning. They can cause addiction and physical dependence, even at prescribed doses.
- Stopping a benzodiazepine suddenly can bring life-threatening withdrawal, seizures among them. It is tapered slowly with a doctor.
- A benzodiazepine with opioids can cause deep sedation, slowed breathing, coma and death. Alcohol and other sedatives add to the danger.
- Two treatments tried and the attacks still heavy: ask for referral to a specialist mental health service.
