Stop Your Hair Loss
Hair loss
Also called Male pattern baldness, Receding hairline, Balding, Thinning hair, MPB
Who it's forA man whose hairline is going back or whose hair is thinning on top.
Tonight
- The first photo set: hairline from the front, crown from above, dry, same light
- Name the pattern: temples and crown, round patches, or shedding all over
- List every medicine and supplement from the months before
- If it is the pattern: book the doctor for finasteride and 5% minoxidil
The plan
22 moves, in order, after 5 checks. Tap any row to see how.

Name the kind of lossName it: pattern, patches or sheddingCheck
Name the kind of loss
For: Everyone losing hair
First work out which kind of hair loss you have. A slow retreat at the temples and crown is the common kind.
How to check
- Look at your temples and crown in good light. Doctors grade the pattern on the Norwood scale.
- Round bald patches, hair falling out all over, or thinning eyebrows: see a doctor first.
- A red, sore or scaly scalp: see a doctor first.
- What it is
- One look
- When
- Before you start
- How often
- Once
Standard practice
Sudden or patchy loss, loss with no family history, loss you cannot explain, or a red or sore scalp: a doctor before the minoxidil under the stat.
Reading the number
Pattern, patches or shedding
Your history and a look at your scalp tell the kinds apart. The common kind usually starts in the teens and slowly progresses.
¹ JAAPA, 2026. The study is under The science, below.
Watch out
A receding hairline with thinning eyebrows, or a scaly or sore scalp, needs a dermatologist early. Scarring loss is permanent.
The science
- In detail
- Look at where it is going. A slow retreat at the temples and the crown, over years, is the pattern, and doctors grade it on the Norwood scale. Round bald patches, hair coming out all over, or a sore, scaly or burning scalp is something else and goes to a doctor first.
- Found
- A clinical review: your history and a look at the scalp tell the kinds apart. Loss all over points to shedding, round patches to alopecia areata, and a pattern to male-pattern loss. Pattern loss usually starts in the teenage years and progresses.
- Why
- Each kind has its own cause. Pattern loss is the follicle shrinking under a hormone. Shedding is many follicles resting at once after a shock. Patches are the immune system attacking the follicle. The treatment follows the cause.
- Watch
- A receding hairline with thinning eyebrows can be frontal fibrosing alopecia, a scarring loss that is permanent once the follicle scars: see a dermatologist early. A scaly patch can be scalp ringworm, which needs antifungal treatment.
- How it is done
- A look in good light; Before any treatment; Once
The studies
History and scalp inspection guide diagnosis, with the differential including such conditions as telogen effluvium (diffuse), alopecia areata (focal), and androgenetic alopecia (patterned).

What a doctor checksHistory, scalp, and thyroid and iron if neededCheck
What a doctor checks
For: Everyone losing hair
A doctor asks when it started, looks at your scalp, and may test your blood for thyroid and iron problems.
How to check
- Note when the loss started and how fast.
- If it is all over the scalp, ask about thyroid and iron tests.
- Take iron only if the test says you are low.
- What it is
- A blood test
- When
- At the first visit
- How often
- Once
Standard practice
A blood test or a scalp biopsy when the doctor suspects a disease, a deficiency, a hormone imbalance or an infection. Iron or zinc only when a test shows a shortfall.
Reading the number
Thyroid and iron, checked
The review lists thyroid, blood count and iron tests. A small skin sample is only for suspected scarring.
¹ JAAPA, 2026. The study is under The science, below.
Watch out
Only take iron if a test shows you are low. Too much is harmful.
The science
- In detail
- Tell the doctor when it started and how fast. Expect a magnified look at the scalp (trichoscopy), a gentle pull on the hair, and blood tests if the story points past pattern loss. A biopsy if scarring is suspected.
- Found
- Thyroid, blood count, iron. A clinical review lists the usual blood tests: thyroid, a full blood count and iron. A scalp biopsy confirms scarring. Trichoscopy, the magnified look at the scalp, and selective testing sharpen the diagnosis.
- Why
- An underactive or overactive thyroid and low iron both thin hair across the whole scalp. Both show on a blood test, and both are treated at the cause.
- Watch
- Take iron or zinc only if a blood test shows you are low. Too much iron is harmful.
- How it is done
- A blood test, if needed; The first visit; Once
The studies
Recommended laboratory tests include thyroid-stimulating hormone, complete blood cell count, and iron studies, while scalp biopsy is used to confirm scarring.

The PSA on finasteridePSA before dose one; tell every doctorCheck
The PSA on finasteride
For: On finasteride
Finasteride lowers the PSA, the blood test for prostate problems. Doctors need to know so they read it right.
How to check
- Get a PSA before your first tablet.
- Tell any doctor who orders a PSA that you take finasteride.
- A rise from your lowest reading gets checked.
- What it is
- A blood test
- When
- Before the first dose
- How often
- Then as your doctor books
Standard practice
On finasteride, any confirmed rise from your lowest PSA gets checked, even inside the normal range.
Reading the number
PSA down 40 to 50%
In a 48-week trial, 1 mg a day lowered the PSA by 40% to 50%. Doctors double the reading to correct for it.
¹ The Lancet. Oncology, 2007. The study is under The science, below.
Watch out
A PSA read without knowing you take it can look falsely low.
The science
- In detail
- A PSA on file before the first tablet. On finasteride the reading runs lower, so tell every doctor who orders a PSA that you take it. A confirmed rise from your lowest reading gets checked.
- Found
- A 48-week trial, 355 people: 1 mg a day cut the PSA by a median 40% to 50%. Placebo readings held or rose. The authors say the doubling rule for the prostate dose applies to the hair dose too.
- Why
- PSA is made by the prostate, and the prostate runs on DHT. Cut the DHT and the prostate makes less PSA, so a normal-looking number can hide a real rise.
- Watch
- A PSA read without knowing you take finasteride looks falsely low.
- How it is done
- A blood test; Before the first dose; Then as the doctor books
The study
who were assigned 1 mg/day finasteride had a median decrease in serum PSA concentration of 40% (95% CI 34-46) and 50% (44-57), respectively

Fertility on finasteridePlanning a child: say so before you startCheck
Fertility on finasteride
For: Planning a child
In the main trial, the hair dose did not change sperm. A few men see lower counts, which mostly recover after stopping.
How to check
- Planning a child soon: tell the doctor before you start.
- Trying and it is slow: ask for a semen test.
- Low counts on it: stopping is the usual advice.
- What it is
- Nothing extra
- When
- Before you start
- How often
- While trying for a child
Reading the number
Sperm unchanged at 48 weeks
In a 48-week trial, 1 mg a day did not affect sperm. In one clinic, counts that had fallen rose 11.6 times on average after stopping.
¹ The Journal of urology, 1999. The study is under The science, below.
Watch out
Trying for a child with low counts: stopping is the usual advice.
The science
- In detail
- If a child is planned soon, say so before you start. In the 48-week trial the hair dose left sperm production alone. A few men see lower counts, and in one clinic’s records most recovered after stopping.
- Found
- A 48-week trial, 181 people, 79 with semen tested: 1 mg a day did not affect sperm production or semen. In one fertility clinic’s records, low-dose finasteride lowered counts in some, and counts rose 11.6-fold on average after stopping.
- Why
- Sperm are made in the testis, which runs on testosterone more than on DHT. Finasteride lowers DHT and leaves testosterone in place, which is why counts mostly hold.
- Watch
- Trying for a child with low counts: stopping it is the usual advice.
- How it is done
- Nothing extra; Before you start; While trying for a child

Mood on finasteride
For: On finasteride
Suicidal thoughts are a confirmed side effect of finasteride. How often it happens is not known, so you watch for it.
How to check
- Before you start, note how your mood usually runs.
- Tell someone close that you have started.
- Any mood change: stop it and call your doctor as soon as you can.
- What it is
- A mood check
- When
- Before and during
- How often
- For as long as you take it
Standard practice
Suicidal thoughts are a confirmed side effect of finasteride 1 mg and 5 mg tablets, frequency unknown. On the 1 mg tablet, any mood change means stop it and contact your doctor as soon as possible. The 1 mg pack now carries a patient card.
Reading the number
Mood change: stop and call
The EU regulator confirmed suicidal thoughts as a side effect, with most reports on the hair tablet. It advises stopping and calling the doctor at any mood change.
¹ Brain sciences, 2026. The study is under The science, below.
Watch out
Any mood change: stop the tablet and call your doctor.
The science
- In detail
- Know how your mood usually runs before you start, and tell someone close that you have started.
- Found
- A confirmed side effect. The EU medicines regulator’s 2025 review confirmed suicidal thoughts as a side effect of finasteride 1 mg and 5 mg. How often it happens is unknown, and most reports were on the 1 mg hair tablet. The authors of a drug-safety analysis advise mood checks before and during. A large long-term study found no link.
- Why
- The enzyme finasteride blocks also makes calming steroids in the brain, which is the proposed route to mood effects. Sexual side effects can also feed a mood change, which is why the regulator asks for both to be reported.
- Watch
- Suicidal thoughts are a confirmed side effect, frequency unknown. Any mood change: stop and call your doctor.
- How it is done
- A mood check; Before and during; For as long as you take it
The studies
A baseline psychiatric assessment and the regular monitoring of mood, sexual function, and suicidal ideation are recommended.

1Shedding after a shockShedding after a shock: usually settlesModerate evidence
Shedding after a shock
For: Sudden shedding
Hair falling out all over, a few months after an illness or a hard time, is usually shedding. It tends to stop by itself.
What to do
- Think back 2 or 3 months: illness, fever, surgery, a crash diet or stress.
- If there was one, give it time.
- Still shedding after 6 months: see a doctor.
- How much
- Nothing
- When
- After a shock
- How long
- 6 to 9 months
Standard practice
Shedding shows a few months after the shock and stops as the body readjusts. The hair tends to be back to normal fullness within six to nine months.
What happened in the studies
Usually stops by itself
The review found this shedding is common and usually settles on its own. Stress is thought to be the most common trigger.
¹ Italian journal of dermatology and venereology, 2023. The study is under The science, below.
Watch out
Shedding past 6 months, or thinning under it, needs a doctor.
The science
- In detail
- Hair coming out all over, two or three months after a fever, an operation, a crash diet or a hard stretch, is probably shedding. It usually settles by itself once the trigger has passed.
- Found
- Usually settles by itself. A review of the condition: this shedding is one of the most common causes of thinning all over the scalp, and it usually settles by itself. Stress is felt to be its most common trigger.
- Why
- A shock tips many follicles into their resting phase at once. They drop their hairs together a few months later. When the body settles, the follicles start growing again.
- Watch
- Shedding that runs past six months, or a thinning pattern under it: a doctor.
The study
Telogen effluvium (TE) is one of the most common presentations of diffuse non-cicatricial alopecia and is generally self-limited.

- Show the list to your doctor.
- Never stop a prescribed medicine without asking.
- How much
- A list
- When
- Before the first visit
- How long
- Once
Standard practice
If you suspect a medicine is behind the loss, talk with your doctor. Stopping a treatment suddenly can cause serious side effects.
What happened in the studies
Hair loss is a listed risk
A review of testosterone taken for muscle lists hair loss among its risks, next to liver damage and breast growth.
¹ International journal of dermatology, 2025. The study is under The science, below.
Watch out
Stopping a prescribed medicine suddenly can be dangerous. Ask first.
The science
- In detail
- List every medicine and supplement you started in the months before the loss. Testosterone or anabolic steroids can bring pattern loss on. Never stop a prescribed medicine on your own.
- Found
- Hair loss, a listed risk. A review of testosterone taken for muscle and strength: the health risks it names are liver toxicity, breast growth and hair loss.
- Why
- Testosterone is the raw material for DHT, the hormone that shrinks follicles on a scalp built to lose hair. More testosterone means more DHT at the follicle. Some medicines stop hair growing until they are stopped.
- Watch
- Stopping a prescribed medicine suddenly can cause serious side effects. Ask first.
The study

- Found
- A standard for hair photos: position, lighting, pose and technique vary so much that most photos cannot be compared. The standard sets nine fixed views, taken the same way every time.
- Why
- Hair changes too slowly to see in a mirror. A fixed photo is the only fair comparison with where you started, and it is how the hair trials judge the result too.

What happened in the studies
45%more regrowth at 5%
In a 48-week trial, the 5% strength grew 45% more hair than the 2% strength.
¹ Journal of the American Academy of Dermatology, 2002. The study is under The science, below.
Watch out
An itchy scalp, and some shedding in the first 2 months. Heart disease: ask a doctor first. Keep it away from cats.
The science
- In detail
- The 5% solution or foam, twice a day on a dry scalp, from the week you notice thinning. Sold without a prescription. With the finasteride, and for as long as you want the hair. Stop, and the new hair goes inside months.
- Found
- 45% more regrowth at 5% than 2%. Forty-eight weeks, 393 people: 5% beat 2% and placebo on hair count and on what the man saw in the mirror, with 45% more regrowth than 2%. Pooled trials: 2.3 times as likely to show visible growth on the investigator’s rating.
- Why
- The scalp enzyme sulfates it into its active form, which opens potassium channels, widens the follicle's blood supply and extends the growth phase. The enzyme varies between scalps, which is why the response varies from man to man.
- Watch
- Scalp irritation and itch, more at 5%. A shed in the first two months, which is the old hairs leaving. Stop it and the gain goes. Ask a doctor first if you have heart disease. Keep it away from cats: a few drops can make a cat critically ill.

- Density and size up. In a long-term comparison against 2% minoxidil, the 2% shampoo improved hair density, hair size and the share of growing follicles almost as much. A review of five human studies, 318 people, found thicker hair shafts.
- Why
- Ketoconazole kills the scalp yeast behind a low-grade inflammation around the follicle, and it calms inflammation on its own. In the trial it also cut the oil on the scalp.
- Watch
- Scalp dryness or irritation.

Watch out
Can irritate the scalp. Never drink it.
The science
- In detail
- Rosemary oil rubbed into the scalp nightly, six months before you judge it. Never swallowed.
- Found
- hair count up at 6 months. Six months, 100 people, against 2% minoxidil: hair count up in both groups at six months, and the rosemary scalp itched less.
- Why
- Its terpenes widen the small vessels around the follicle, the same lever minoxidil pulls with a drug. Route decides everything: on the scalp it is a tonic, by mouth an essential oil is a poison.
- Watch
- Scalp irritation. Never by mouth.

7Low-Level Light TherapyRed light on the scalp, 4 to 26 weeksStrong evidence
Low-Level Light Therapy
For: Hair loss
Red light gives hair roots more energy, so hair grows thicker.
What to do
- Use a red-light cap or helmet on your scalp, on the schedule it comes with.
- Keep your eyes out of the beam.
- Give it 4 to 26 weeks.
- How much
- The device’s schedule
- When
- Per device
- How long
- 4 to 26 weeks
What happened in the studies
Thicker hair, more past 20 weeks
Across 38 studies, red light grew clearly thicker hair than a fake light. A 24-week helmet trial found the same.
¹ Dermatologic surgery, 2025. The study is under The science, below.
Watch out
Never shine it in your eyes. A short shed and an itchy scalp can happen.
The science
- In detail
- A red-light cap, helmet or comb on the scalp, on the schedule its own trial ran, for four to twenty-six weeks before you judge it. Beside the drugs, or for the man who will not take them.
- Found
- Large effect after 20 weeks. Thirty-eight studies, 3,098 people: hair density up against a fake treatment light, a large effect before 20 weeks and a larger one after. A 24-week home-helmet trial against a fake treatment helmet: density and hair diameter both up.
- Why
- Red and near-infrared light is absorbed by an enzyme in the mitochondria and briefly lifts a brake on the cell’s energy output. In the follicle that reads as a longer growth phase.
- Watch
- Never into the eyes. The beam that grows hair injures a retina. A short shed and an itchy scalp were the side effects reported.

8AdenosineAdenosine 0.75% lotion, twice a day, with minoxidilModerate evidence
Adenosine
For: Hair loss, on minoxidil
A scalp lotion that makes thin hairs grow thicker. It sits beside minoxidil, not in place of it.
What to do
- Put an adenosine lotion on the thinning area twice a day.
- Keep your minoxidil going.
- Give it 6 months.
- How much
- A lotion
- When
- Twice a day
- How long
- 6 months to judge
What happened in the studies
Thicker hairs at 6 months
In a 6-month trial, adenosine raised the share of thick hairs and hair density against a dummy lotion. All 7 trials reviewed found less hair loss.
¹ The Journal of dermatology, 2016. The study is under The science, below.
Watch out
Can irritate the scalp. A short shed at the start.
The science
- In detail
- An adenosine lotion on the thinning scalp twice a day, beside the minoxidil. The pooled trials used 0.75%. Expect thicker hairs before you expect more of them. Judge it at six months.
- Found
- Seven trials of adenosine lotion or shampoo: every one reported less hair loss and more density. In a 6-month placebo trial, the share of thick hairs rose, fine hairs fell, and density rose against placebo.
- Why
- Adenosine docks on receptors in the dermal papilla, the cells at the base of the follicle that set how thick a hair grows. They answer with growth factors that keep the follicle building and feed it more blood.
- Watch
- Scalp irritation. A short shed when you start.

The science
- In detail
- The low-dose pill, prescribed off-label for hair, for the man the foam did not suit. The consensus start is 2.5 mg a day, within 1.25 to 5 mg. Blood pressure, weight and heart history are checked first.
- Found
- +47 hairs per cm² per mg a day. Six studies, meta-regression: each extra milligram a day added 47 hairs per square centimeter and 1.4 microns of shaft at six months, 18 points of hypertrichosis risk and 4.8 points of cardiovascular side effects. A review: unwanted hair in 24%, an early shed in 16 to 22%, mild swelling in 2%, pericardial effusion rare at hair doses.
- Why
- Minoxidil is switched on by an enzyme in the follicle. It opens potassium channels that widen the blood supply and hold the growth phase. Swallowed, it sends the signal to every follicle on the body. That is how it works and the side effect in one.
- Watch
- Unwanted hair on the face and body in about a quarter. An early shed in 16 to 22%. Fluid swelling in about 2%. Cardiovascular side effects rise with the dose. Pericardial effusion, fluid around the heart, is rare at hair doses. Not with pericardial disease or uncontrolled blood pressure.

Watch out
Redness for a day or two. Wait if your scalp is red or infected.
The science
- In detail
- Microneedling on the thinning area once a week, on top of 5% minoxidil twice a day. With a dermatologist, or on one’s advice, with a device made for the scalp.
- Found
- A review of twelve trials, 631 people: microneedling plus minoxidil grew more hair, and thicker hair, than minoxidil alone. Side effects were mild or passed on their own. In eight pooled trials, every one showed a higher hair count on the combination.
- Why
- The tiny wounds set off the skin’s repair signals and growth factors around the follicle. They may also let more minoxidil into the scalp.
- Watch
- Redness and soreness for a day or two. A red, infected or inflamed scalp is a reason to wait.
The studies

The science
- In detail
- Medical tattooing of tiny dots that read as cut stubble, for the man whose goal is to look fuller. Three weekly sessions and a touch-up a month later, in a medically supervised setting. It changes how the scalp looks, not the hair.
- Found
- A series of 22 people: three weekly sessions and a touch-up a month later. Over a mean of 20 months fading was minimal, there was one allergic reaction and no serious complications.
- Why
- Thinning shows because pale scalp shows between the hairs. Pigment dots in the upper skin close that contrast, so the hair that is there reads as denser.
- Watch
- An allergic reaction to the pigment is possible.

¹ Journal of the American Academy of Dermatology, 1998. The study is under The science, below.
Watch out
Sexual side effects about 1.7 times more common than on a dummy pill, sometimes lasting after stopping. Report any breast lump or pain. The label warns of a higher risk of high-grade prostate cancer. Any mood change: stop and call your doctor.
The science
- In detail
- One milligram a day, the tablet approved for male-pattern hair loss, on a prescription. Start the year you notice the hairline or the crown move, and take it for as long as you want the hair. A PSA on file before dose one.
- Found
- 1,553 people; loss slowed at 1 mg. Two one-year trials, 1,553 people, extended to two: 1 mg a day slowed the loss and grew hair by every measure. The dose-ranging trials: 0.2 mg works, and 1 and 5 mg work the same. Fifteen placebo trials, 4,495 people: sexual dysfunction at 1.66 times the placebo rate on finasteride.
- Why
- It blocks type 2 5-alpha-reductase, the enzyme that makes DHT from testosterone. DHT is what shrinks the follicle on a scalp built to lose hair. Lowering DHT is the whole of how it works, on the scalp and everywhere else it acts.
- Watch
- Sexual dysfunction at 1.66 times the placebo rate, and on the label, sexual problems that continued after stopping. Report any breast lump, pain or nipple discharge promptly. The label warns this drug class may raise the risk of high-grade prostate cancer. Suicidal thoughts are a confirmed side effect: any mood change, stop and call the doctor. It lowers the PSA, so the baseline goes on file first. Never handled by a pregnant partner.

19DutasterideDutasteride 0.5 mg, if finasteride has not heldStrong evidence
Dutasteride
For: Hair loss, past finasteride
A stronger cousin of finasteride. It blocks more of the hormone that shrinks hair roots.
What to do
- Ask a doctor if finasteride has not held your hair. For hair, it is prescribed off-label.
- Take 0.5 mg once a day.
- Get a PSA on file first.
- How much
- 0.5 mg
- When
- Once a day
- How long
- 24 weeks to judge
Standard practice
A pregnant partner does not handle the capsules. No blood donation until at least 6 months after the last dose. Its half-life is about 5 weeks, and it stays detectable in the blood for 4 to 6 months after stopping.
What happened in the studies
More hair than finasteride
In a 24-week trial of 917 people, 0.5 mg a day grew more hair than finasteride. Sexual side effects were about the same.
¹ Journal of the American Academy of Dermatology, 2014. The study is under The science, below.
Watch out
Sexual side effects, about as often as on finasteride. The label warns of a higher risk of high-grade prostate cancer. Any mood change: call your doctor.
The science
- In detail
- A stronger blocker of the same hormone, 0.5 mg a day, to ask a doctor about when finasteride has not held the hair. In the US it is licensed for the prostate and prescribed off-label for hair. Japan approves it for hair loss.
- Found
- Beat finasteride 1 mg at 24 weeks. 917 people, 24 weeks: hair count and width rose with the dose, and 0.5 mg beat finasteride 1 mg on count, width and the frontal photos. Three head-to-head trials, 576 people: more hair than finasteride, with similar sexual side effects.
- Why
- DHT is made by two forms of the enzyme. Finasteride blocks one, dutasteride blocks both, so DHT falls further in the scalp and the blood.
- Watch
- Sexual side effects at rates like finasteride’s. The label warns it may raise the risk of high-grade prostate cancer. It lowers the PSA too, so the baseline goes on file first.
The studies
Dutasteride 0.5 mg significantly increased hair count and width in a 2.54-cm diameter and improved hair growth (frontal view; panel photographic assessment) at week 24 compared with finasteride

20Finasteride on the scalpFinasteride spray, with a dermatologistModerate evidence
Finasteride on the scalp
For: Hair loss
Finasteride as a scalp spray. It works on the scalp with much less of the drug in your blood.
What to do
- Ask a dermatologist. It is authorized in the EU, not in the US.
- Use it on the thinning area as prescribed.
- Give it 24 weeks.
- How much
- As prescribed
- When
- As prescribed
- How long
- 24 weeks to judge
Standard practice
A woman who is or may become pregnant, and anyone under 18, should not touch the spray, the treated scalp or anything it has reached. If they do, wash the area well.
What happened in the studies
Tablet-level hair, less in blood
In a 24-week trial of 458 people, the spray grew about as much hair as the tablet, with blood levels over 100 times lower. Sexual side effects came in 2.8% on the spray against 4.8% on the tablet.
¹ Journal of the European Academy of Dermatology and Venereology, 2022. The study is under The science, below.
Watch out
Sexual side effects in about 3 in 100 on the spray, against about 5 in 100 on the tablet. See your doctor about any low mood or any breast lump or pain. Tell the doctor before a PSA test. Pregnant women and anyone under 18 should not touch the spray or the treated scalp.
The science
- In detail
- Finasteride as a scalp spray, for the man who wants the drug with less of it in his blood. Ask a dermatologist. In the EU a finasteride skin spray is authorized for mild to moderate pattern loss. The US has no approved topical form, and its regulator has warned about compounded ones.
- Found
- 20.2 hairs against 6.7. 458 people, 24 weeks: hair count up 20.2 hairs against 6.7 on placebo, and about level with the tablet. Blood DHT fell 34.5% against 55.6% on the tablet, and peak drug levels in the blood were over 100 times lower. Across the pooled trials, sexual side effects came in 2.8% on the 0.25% spray against 4.8% on the tablet.
- Why
- On the scalp, the drug blocks DHT where the follicle is. Less reaches the blood, so the rest of the body keeps more of its DHT.
- Watch
- Sexual side effects in 2.8% on the spray against 4.8% on the tablet, and some have lasted after stopping. The label says to see the doctor about any low mood or other psychiatric symptom. Report any breast change promptly: a lump, pain, swelling or nipple discharge. There are no data yet on its effect on the PSA, so tell the doctor you use it before a PSA test. It is not for women. A woman who is or may become pregnant, and anyone under 18, should not touch the spray, the treated scalp or anything it has reached. If they do, wash the area well.
The studies
Topical finasteride significantly improves hair count compared to placebo and is well tolerated.

21PRP on the scalpPRP, a monthly course, then top-upsModerate evidence
PRP on the scalp
For: Hair loss
A clinic spins your own blood and puts the part rich in growth factors into your scalp.
What to do
- Ask a dermatologist if PRP suits you.
- Expect a session once a month for 3 months.
- Then a top-up every 3 to 6 months.
- How much
- One session
- When
- Monthly, then top-ups
- How long
- 6 months to judge
Standard practice
Most people return once a month for 3 months, then once every 3 to 6 months.
What happened in the studies
Thicker hair at 6 months
Across 9 trials, PRP made hair thicker at 3 and 6 months than a dummy treatment. No serious side effects were found.
¹ Journal of cutaneous medicine and surgery, 2023. The study is under The science, below.
Watch out
Pain, swelling or a bruise where it goes in.
The science
- In detail
- Your own blood, spun down in a clinic, and the plasma rich in platelets placed into the thinning scalp by a doctor. A course at first, then top-ups. Beside the drugs.
- Found
- Density up at 3 and 6 months. Nine trials, 238 people: hair density up at 3 and 6 months against placebo, with no serious side effects. In a network of 27 trials, 1,110 people, PRP added to 5% minoxidil ranked above minoxidil alone.
- Why
- Platelets carry growth factors. Concentrated and placed at the follicle, they are thought to push it into its growth phase and keep it there.
- Watch
- Pain, swelling, bruising or redness where it goes in, for a day or two.
The studies
PRP is an effective and safe treatment for increasing the hair density in AGA.

22Hair transplantA transplant consultation, once the loss is steadyModerate evidence
Hair transplant
For: Hair loss, steady on drugs
A surgeon moves hair roots from the back of your head to the thin areas.
What to do
- Get the loss steady on the drugs first.
- See a hair-restoration surgeon for a consultation.
- Keep taking the drugs after, for the hair that was not moved.
- How much
- One procedure
- When
- Once the loss is steady
- How long
- Months to grow in
Standard practice
A dermatologist may raise a transplant for an area of pattern thinning or balding.
What happened in the studies
Your own hair, moved
In a before-and-after study, people felt better about themselves and their lives after the transplant.
¹ Dermatology online journal, 2014. The study is under The science, below.
Watch out
It is surgery, with swelling and scabs while it heals.
The science
- In detail
- Once the loss is steady on the drugs, a consultation with a hair-restoration surgeon. Follicles are moved from the back of the head to the thin areas. Keep the drugs going after, for the hair that was not moved.
- Found
- Your own follicles, moved. Follicular unit excision takes single follicle groups from the back of the scalp with a small punch and moves them into the areas of loss. In a before-and-after study, self-esteem and quality of life were both better after the surgery.
- Why
- Follicles at the back and sides are built to ignore DHT, and they keep that trait when moved. The moved hair lasts. The hair around it still needs the drugs.
- Watch
- It is surgery: swelling, scabbing and a healing period. It moves hair rather than making more, so the donor supply sets the limit.
The studies
Follicular Unit Extraction (FUE) is a minimally invasive surgical procedure that utilizes a punch device to harvest occipital follicular units that are later transplanted in areas of loss to restore hair growth.

How you'll know it's working
- Track
- Hair density, from the photo
- How to track
- The same photo set every three months: same light, same angle, hair dry, the crown from above and the hairline from the front. Score it 1 to 10 against the first, and note the Norwood stage.
- When to judge
- A shed on minoxidil can come in the first two months. The drugs start to show by month four. Judge at six months, and the photo convinces at a year.
Why this happens
The hairline in the bathroom mirror is not where it was in the photo from three summers ago. The crown shows under the lights at the gym. Or the shower drain holds more than it used to, and you cannot tell if it is the start of something or a bad month.
The hormone
On a scalp built to lose hair, DHT shrinks the follicle. DHT is the strong androgen an enzyme makes from testosterone. Each cycle the hair grows back finer and shorter, until it stops. Finasteride and dutasteride cut the DHT. That is the lever that holds the hair.
The growth phase
Each hair grows for years, rests, then falls. Minoxidil widens the follicle’s blood supply and holds the growth phase open. Red light, microneedling and PRP push the same way from outside. That is the lever that thickens it. Stop it and the gain goes.
The other causes
Not every loss is the hormone. A fever, an operation or a crash diet can tip many follicles into rest at once, and they shed together months later. Round patches are the immune system. A thyroid problem or low iron thins the whole scalp. Each is fixed at its cause.
Week by week
Why this order
The two drugs first, then one addition at a time, so you know which one is working. Where a label sets a dose, follow the label.
| When | Add | Why then |
|---|---|---|
| Week 0 | The first photo set. Name the pattern. List what you take. Patches, shedding all over, or a sore scalp: the doctor first, before any drug. | The treatment follows the cause, and the photo is what every later move is judged against. |
| Week 0, if it is the pattern | The doctor this month: finasteride 1 mg with a PSA on file first, and 5% minoxidil twice a day. Both together, for as long as you want the hair. | They pull different levers, so they start together. Hair held is far easier than hair regrown. |
| Weeks 2 to 8 | Ketoconazole shampoo a few times a week. Expect a shed on minoxidil in the first two months. | The shed is the old hairs leaving. It is the drug starting, not failing. |
| Weeks 8 to 26 | Rosemary oil or a red-light device, one at a time. Microneedling with the minoxidil, on a dermatologist’s advice. | Narrower moves, added one at a time so a change in the photo can be credited. |
| Week 16 on, if you want more | The early-research extras, one at a time, beside the drugs: a caffeine scalp product, pumpkin seed oil capsules or a bFGF serum. Scalp micropigmentation any time the goal is looking fuller. | Small or first studies pointing the right way. Each one judged on the same photo. |
| Week 26, the review | The second photo set against the first. Shedding slower, crown denser, hairline held: keep going. Not moving: dutasteride, oral minoxidil or PRP, with the doctor. | Six months is the first honest read on the drugs. The labels ask for months, not weeks. |
| Week 52, the photo | The one-year photo set. Steady, and still wanting more at the front: a hair-restoration consultation. | A transplant moves hair onto a scalp the drugs are already holding. |
| Any week | Low mood, dark thoughts or a sexual change on finasteride: stop and call the doctor. New patches or a sore scalp: the doctor. | Lines that take a man off this page. Neither waits for the review. |
What to track
One number: hair density, from a standard photo, scored 1 to 10 against the first, with the Norwood stage noted, every three months. Same light, same angle, dry hair, the crown from above and the hairline from the front, the phone under a stat and not in a hand. Hair is slow. The shed can come early, the drugs show by month four, and the photo convinces at a year.
The diary is a week: whether the tablet went in, whether the minoxidil went on twice, the shampoo days, and anything that changed in mood or sex. Where the photo has not moved by the review, the order above says what to do next.
The diary, one week
| Day | Photo taken? | Finasteride taken | Minoxidil, twice | Shampoo day | Mood, sex: any change |
|---|---|---|---|---|---|
| Mon | |||||
| Tue | |||||
| Wed | |||||
| Thu | |||||
| Fri | |||||
| Sat | |||||
| Sun |
- Hair density, from the photo: judged at six months and a year, never at six weeks. The first two months on minoxidil can shed more hair than they grow. That is the old hairs leaving.
- Days on the drugs out of seven: the one column that predicts the photo. Missed days are the most common reason the photo does not move.
- Mood and sex, one line a week on finasteride or dutasteride. A change is a call to the doctor, not a column to watch.
- A PSA on file before the first tablet, and every doctor told that you take it.
- Lines that take a man off this page and to a doctor: new round patches, a red or sore scalp, or dark thoughts on finasteride.

- A randomized controlled trial: people put by chance on the treatment or on a dummy version.
- PMID
- A study's ID number in PubMed, the public index of medical research.
- Strong, moderate and early evidence
- Strong: many human trials agree. Moderate: human studies point the same way. Early: the first small human studies.
- Androgenetic alopecia
- Pattern hair loss: the follicle shrinks under DHT on a scalp built to do so. The hair this page holds.
- DHT and 5-alpha-reductase
- Dihydrotestosterone, the strong androgen made from testosterone by the enzyme 5-alpha-reductase. Finasteride blocks one form of the enzyme, dutasteride both.
- Telogen effluvium
- Shedding all over the scalp a few months after a shock, as many follicles rest at once. It usually settles by itself.
- Alopecia areata
- Round bald patches from the immune system attacking the follicle. A doctor’s problem, not this page’s.
- Terminal hair
- The thick, pigmented hair, against vellus, the fine down. Terminal count per square centimeter is what the hair trials measure.
- Hypertrichosis
- Hair growing where it was not wanted: the face, the arms, the back. Oral minoxidil’s signature side effect.
- PSA
- Prostate-specific antigen, the blood test for prostate problems. Finasteride and dutasteride lower it.
- Off-label
- A licensed drug prescribed for a use its label does not list. Legal on a doctor’s judgment.
- Microneedling
- A roller or pen of fine needles that makes tiny wounds in the scalp to set off repair.
- PRP
- Platelet-rich plasma: your own blood, spun so the platelets concentrate, then placed in the scalp.
The first photo tonight. Name the pattern before you treat it. If it is the pattern, the two drugs go in this month, together, because hair held is hair kept. Everything else on this page is added one at a time and judged at six months against the photo.
When to see a doctor
- Hair loss that is sudden or comes out in patches.
- A scalp that is red, inflamed, infected, irritated or painful.
- Hair loss with no family history of it, or with no reason you know of.
- A receding hairline with thinning or lost eyebrows. This can be a scarring loss, and early treatment can stop it.
- A scaly patch on the scalp. Scalp ringworm needs antifungal medicine.
- Shedding all over after losing 20 pounds or more, a high fever, an operation or an illness.
- Hair loss you think a medicine is causing. Ask before you stop the medicine.
I am not a doctor. This is not medical advice. This is self-education. Every dose and strength on this page is the one a trial used, quoted from the trial. It is not a prescription. Talk to a physician who has your history before anything in the With a doctor group. Keep finasteride and dutasteride away from a pregnant partner.
