Lift Your Mood
Feeling down
Also called Low mood and depression, Low mood, Depression, Feeling flat, Nothing feels good
Who it's forA man who has felt flat, low or down most days for weeks, or who wants to feel good again, not just less bad.
Tonight
- Take the PHQ-8 and write the score down
- Plan one thing you used to enjoy for tomorrow, at a set time
- Set tomorrow’s wake time, and no drink tonight

The PHQ-8 mood scoreThe number to track: eight questions, every two weeksCheck
The PHQ-8 mood score
For: Knowing where you stand
An 8-question score that shows how low you are, and whether it is lifting.
How to check
- Answer the 8 questions about the last 2 weeks.
- Write the score down.
- Repeat every 2 weeks and take the scores to the doctor.
8 questions
Over the last 2 weeks, how often have you been bothered by any of the following problems?
- Little interest or pleasure in doing things
- Feeling down, depressed, or hopeless
- Trouble falling or staying asleep, or sleeping too much
- Feeling tired or having little energy
- Poor appetite or overeating
- Feeling bad about yourself -- or that you are a failure or have let yourself or your family down
- Trouble concentrating on things, such as reading the newspaper or watching television
- Moving or speaking so slowly that other people could have noticed? Or the opposite -- being so fidgety or restless that you have been moving around a lot more than usual

Standard practice
Review how a treatment is working 2 to 4 weeks after starting it. Track it with a standard score such as the PHQ-9.
Reading the number
Tracks the trend
In 6,000 people, a score of 10 or more picked up major depression 88% of the time.
¹ Journal of general internal medicine, 2001. The study is under The science, below.
The science
- In detail
- Eight questions about the last two weeks, each scored 0 to 3. Take it today, then every two weeks, and bring the scores to the doctor.
- Found
- Score of 10: 88% caught. 6,000 people took it, and 580 were checked against a full interview. A score of 10 or more caught major depression 88% of the time. Scores of 5, 10, 15 and 20 mark mild, moderate, moderately severe and severe.
- Why
- Low mood hides its own trend. A score on paper every two weeks shows whether the moves below are working, and gives the doctor a number instead of a feeling.
- How it is done

Train hard, most days
For: Everyone
Exercise is one of the strongest things you can do for low mood. Hard, regular and with other people works best.
What to do
- Move most days, hard enough that talking is work.
- Lift weights twice a week.
- Train with a friend, a class or a coach if you can.
- How much
- Twice a week lifting
- When
- Most days
- How long
- Every week
Standard practice
A structured exercise program for low mood usually runs more than once a week for 10 weeks, at moderate intensity, in a group.
What happened in the studies
Less depression
Across 1,079 trials, exercise cut depression, aerobic work most. The biggest gains were at ages 18 to 30.
¹ British journal of sports medicine, 2026. The study is under The science, below.
Watch out
Injuries from poor form. Build up gradually.
The science
- In detail
- Move most days, hard enough that talking is work: walking fast, jogging, lifting, a class. Lift twice a week. Training with other people, or with a coach, did the most.
- Found
- Medium effect on depression. An umbrella of 1,079 trials and 79,551 people: exercise cut depression by a moderate effect, aerobic work the most. The biggest gains were at ages 18 to 30, and in group or supervised sessions. Across 218 trials, walking or jogging, yoga and lifting led.
- Why
- Training raises the growth signals the brain uses to remodel, burns down the day's stress hormones, and gives the next morning's HRV something to rise from. It is the one move on this page that also fixes the sleep.

2Schedule the good thingsPlan one enjoyable and one avoided thing a day, then do themStrong evidence
Schedule the good things
For: Stopped doing things
Plan the things you used to enjoy back into your week, and do them before you feel like it. The mood follows.
What to do
- Each evening, plan tomorrow: one thing you used to enjoy, one task you have avoided.
- Give each a set time.
- Do them whatever your mood, and rate the mood before and after.
- How much
- Two things a day
- When
- At a set time
- How long
- Every day
Standard practice
Behavioral activation links what you do to how you feel, then plans practical changes that cut avoidance.
What happened in the studies
Less depression, held a year
Across 105 trials with 13,933 people, scheduling activities cut depression, and the gain held at 12 months. Done alone, it still worked.
¹ Clinical psychology review, 2026. The study is under The science, below.
The science
- In detail
- Plan the week in advance: one thing you used to enjoy and one small task you have been avoiding, each day, at a set time. Do it whether or not you feel like it. Note the mood before and after.
- Found
- Medium effect over the comparison group. 105 trials, 13,933 people: behavioral activation beat control conditions by a moderate to large effect, and the gain held at twelve months. Done alone from a book or an app, it still worked.
- Why
- Low mood makes you drop the things that used to feed it, and dropping them deepens the low. Scheduling them back in breaks the loop from the action side. The mood follows the doing.

- Wake at the same time every day.
- If you cannot sleep, run Deeper sleep first.
- How much
- 7 to 8 hours
- When
- Same wake time daily
- How long
- For good
What happened in the studies
Less depression and anxiety
In 65 trials with 8,608 people, better sleep brought depression and anxiety down. The more sleep improved, the more mood did.
¹ Sleep medicine reviews, 2021. The study is under The science, below.
Watch out
Sleeping 9 hours or more and still tired is a question for a doctor.
The science
- In detail
- Seven to eight hours, the same wake time seven days a week. If you are not sleeping, run Deeper sleep first. This page rides on it.
- Found
- g = 0.63 on depression. Sixty-five trials, 8,608 people: when a treatment improved sleep, depression fell by a medium effect. Anxiety and brooding fell with it. The more sleep improved, the more mood did.
- Why
- A short night raises the amygdala's reactivity and cuts the prefrontal brake on it the next day. Regular sleep is the one move here that every other move depends on.
- Watch
- Nine hours or more a night that does not refresh is a doctor's question, not a reason to sleep in.
The study
Improving sleep led to a significant medium-sized effect on composite mental health (g+ = -0.53), depression (g+ = -0.63), anxiety (g+ = -0.51), and rumination (g+ = -0.49)

4Drinking less without quittingCount the drinks, then cut a levelStrong evidence
Drinking less without quitting
For: Everyone
Alcohol feels like relief tonight and drags your mood down tomorrow.
What to do
- Count your drinks for a week.
- Then cut back a level, from most nights to some.
- Try a month off if the count surprises you.
- How much
- One level less
- When
- Every week
- How long
- A year
What happened in the studies
Better mental health
In 1,383 people followed for a year, every step down in drinking came with better mental health. A bigger cut meant a bigger gain.
¹ Alcoholism, clinical and experimental research, 2017. The study is under The science, below.
Watch out
If you drink every day and shake without it, stopping suddenly is a medical danger. See a doctor first.
The science
- In detail
- Count the drinks for a week, then cut a level: from most nights to some, from some to few. The smaller glass, every round. A month off if the count surprises you.
- Found
- 1,383 people, one year. 1,383 people, one year: every step down the risk-drinking ladder came with fewer alcohol consequences and better mental health, at the end of treatment and a year on. A bigger step meant a bigger gain.
- Why
- Alcohol sedates on the way down and rebounds on the way up. It breaks the second half of the night, drops the next morning's HRV, and leaves the anxiety it promised to take. Cutting the count gives the mood its mornings back.
- Watch
- Physical dependence, where cutting abruptly is a medical event and not a plan
The study
Any reduction in […] risk drinking level during treatment was associated with significantly fewer alcohol-related consequences and improved mental health at the end of treatment and for up to 1 year posttreatment.

- Found
- 23 trials, 1,120 people. Twenty-three trials, 1,120 people: light therapy beat the comparison treatments on depression scores, a mild-to-moderate effect, outside the winter kind of low mood.
- Why
- Light in the first hour sets the clock in the hypothalamus, and the same pathway runs into the circuits that set mood. A late, weak clock is a flat morning.
- Watch
- Mania or hypomania switch in bipolar disorder

Watch out
Mild stomach upset or headache. Stay at the trial dose. On an antidepressant or a blood thinner, or with bipolar disorder, ask a doctor first.
The science
- In detail
- 30 mg a day of a standardized stigma extract, as 15 mg twice a day, for six weeks. A named standardized extract or nothing: saffron is the most adulterated spice there is.
- Found
- 4.4 points off BDI, 14 trials. Thirty-four trials, 1,769 people in all. On the Beck scale, 14 trials and 817 people: depression down 4.4 points against placebo. Head to head with SSRIs in eight trials, saffron matched the SSRIs, with fewer side effects.
- Why
- Crocin and crocetin carry it, and crocetin is small enough to cross into the brain. The trials measure it on the self-rated scales more than on the doctor's, which is where the effect sits.
- Watch

12St. John's Wort900 mg a day, cleared against your other pillsModerate evidence
St. John's Wort
For: Mild low mood
A herbal extract that lifts mild to moderate low mood.
What to do
- Check it against every other medicine you take, first.
- Take 900 mg of standardized extract a day, in 3 doses.
- Give it 6 weeks.
- How much
- 900 mg
- When
- 3 doses a day
- How long
- 6 weeks
Standard practice
It can weaken many medicines, including seizure drugs, digoxin, some cancer drugs, warfarin and simvastatin. Tell the doctor before you take it.
What happened in the studies
Matched antidepressants
In 29 trials with 5,489 people, it beat a dummy pill and matched standard antidepressants, with fewer people quitting over side effects.
¹ The Cochrane database of systematic reviews, 2008. The study is under The science, below.
Watch out
It stops many medicines working: seizure drugs, digoxin, simvastatin, some cancer drugs, blood thinners, HIV and transplant drugs, and some antidepressants. Dangerous on top of an SSRI. You burn faster in the sun.
The science
- In detail
- The standardized extract, 900 mg a day in three doses, for six weeks, for mild to moderate low mood. Clear it against every other pill you take first: it switches on the liver enzymes that clear half the pharmacy. Never with an antidepressant.
- Found
- 29 trials, 5,489 people. Twenty-nine trials, 5,489 people: the extract beat placebo on response and matched the standard antidepressants, with fewer dropouts for side effects. Thirty-five trials, 6,993 people: more responders than placebo, and it matched antidepressants in mild and moderate low mood.
- Why
- Hyperforin blocks the reuptake of several transmitters at once, which is the antidepressant action. The same molecule induces the liver's drug-clearing enzymes, which is why the interaction list is the longest on this page.
- Watch
- It weakens many medicines: seizure drugs such as phenytoin and carbamazepine, digoxin, simvastatin, some cancer drugs, warfarin, HIV and transplant drugs, and some antidepressants. Serotonin syndrome on top of an SSRI. Sun sensitivity.

The science
- In detail
- 800 mg twice a day, for six weeks. Never on top of an SSRI without your doctor's say-so: it is a serotonin drug in all but name.
- Found
- Medium effect over placebo. Twenty-three trials, 2,183 people: SAMe alone beat placebo on depressive symptoms by a moderate effect, with dropouts no higher. Added to an SSRI that had failed: 36% responded against 18% on placebo.
- Why
- It is the body's methyl donor, feeding the synthesis of the transmitters the antidepressants act on. That is also why it can push serotonin too far in company.
- Watch
- Serotonin syndrome with drugs that raise serotonin. Mania or hypomania, even without a bipolar diagnosis. Gut upset.

A guided quest built on the methods tested in trials, done on your own: six lessons over 66 days. Log your days, plan what matters, and break the hard tasks down.
What to do
- Lesson 1: log what you do each hour, and rate your mood out of 10.
- Lesson 2: plan one thing that matters to you each day, and do it.
- Lesson 3: break the task you have been avoiding into small steps, easiest first.
- Lessons 4 to 6: test the low thoughts, solve one problem at a time, and write your plan for a bad week.
- Score the PHQ-8 every 2 weeks as you go.

- Six lessons
- When
- A lesson every five days, plus a daily log
- How long
- 66 days
Start the quest: Get Out of the Rut
Standard practice
Structured self-help built on these methods usually runs 6 to 8 regular sessions.
What happened in the studies
1in 8 helped

¹ JAMA psychiatry, 2017. The study is under The science, below.
Watch out
PHQ-8 of 15 or more, or climbing: see the doctor before you carry on.
The science
- In detail
- Start the guided quest, built on the methods tested in trials and done on your own: a lesson every five days or more, and a short log every day. Lesson 1, see how mood follows doing. Lesson 2, plan one thing that matters to you each day, and do it. Lesson 3, break the task you have been avoiding into small steps. Then catch and test the low thoughts, solve one problem at a time, and write your plan for a bad week.
- Found
- Thirteen trials, 3,876 people, pooled person by person: the self-guided online course lowered depression and nearly doubled the odds of a response, one extra person better for every 8 who did it. The more of it people finished, the better they did. Across 39 trials, courses with a guide and courses done alone both beat control, and at 6 and 12 months they matched.
- Why
- Low mood runs on a loop: you do less, so less goes right, so you feel worse. The log shows the loop in your own week, and the plan and the small steps break it from the doing side. The skills stay with you when the course ends.
- Watch
- A PHQ-8 of 15 or more, or a score that climbs two check-ins running, is a reason to see the doctor before you carry on.
The studies
Self-guided iCBT was significantly more effective than controls on depressive symptoms severity (β = -0.21; Hedges g = 0.27) and treatment response (β = 0.53; odds ratio, 1.95; 95% CI, 1.52-2.50; number needed to treat, 8).

3.2pointsoff depression
In a trial of 525 people, an online self-help course that taught this cut depression scores by 3.2 points.
¹ BMJ (Clinical research ed.), 2004. The study is under The science, below.
The science
- In detail
- When the mood drops, write down the thought that came with it, word for word: "I always mess it up." Under it, list the evidence for it and the evidence against it. Then write a fairer line, the one you would say to a friend. On paper or in a self-help website built on the methods tested in trials, a few times a week.
- Found
- A trial of 525 people: an online self-help course that taught this cut depression scores by 3.2 points against a control, and a plain information site about low mood worked too.
- Why
- Low mood speaks in absolutes: always, never, nobody. Writing the thought down turns it from a fact into a claim, and a claim can be checked. The fairer line takes the charge out of it.
The study
Both cognitive behaviour therapy and psychoeducation delivered via the internet are effective in reducing symptoms of depression.

A boost on top
In a large network of trials, tryptophan added to an antidepressant beat the antidepressant alone.
¹ Psychological medicine, 2025. The study is under The science, below.
Watch out
With an antidepressant, only with the prescriber: too much serotonin is dangerous. Drowsiness and nausea in some. A rare muscle, skin and lung illness was once linked to tryptophan supplements, most likely from impurities.
The science
- In detail
- Early research. Tryptophan, the amino acid the brain makes serotonin from, added to an antidepressant only with the prescriber. The 0.14 to 3 g a day range comes from a different kind of trial, mood studies of tryptophan on its own, not from the add-on trials.
- Found
- Beat the antidepressant alone. In a network of 192 trials, tryptophan added to an antidepressant beat the antidepressant alone on depression.
- Why
- It is the raw material for serotonin, and only the fraction not diverted down the kynurenine route reaches it.
- Watch
- With an SSRI or another serotonin drug it adds to the serotonin load: serotonin syndrome (agitation, fever, sweating, shaking, a fast heart) is the risk, so only with the prescriber. Drowsiness and nausea in some. An outbreak of eosinophilia-myalgia syndrome, a rare inflammatory illness of the muscles, skin and lungs, was linked to tryptophan supplements; impurities from how it was made are the suspected cause.

Half as likely to get help
For the same symptoms, men were about half as likely to have a diagnosis. What got men in: plain information and role models.
¹ Journal of affective disorders, 2022. The study is under The science, below.
The science
- In detail
- Book the doctor. Take the diary with you: the PHQ-8 scores, the sleep, the drinks, the weeks. Ask what the UK’s NICE guideline offers at your score before you ask for a pill.

31SSRI antidepressantAn SSRI on a prescription, reviewed in the first weeksStrong evidence
SSRI antidepressant
For: Moderate to severe low mood
The standard first antidepressant. It takes weeks to work and needs a doctor watching the first month.
What to do
- Ask the doctor what an SSRI would do for you.
- Take it daily. Expect a review within 1 to 2 weeks.
- Give it 4 weeks. Once it works, stay on it at least 6 months.
- How much
- Sertraline 50 mg
- When
- Once a day
- How long
- At least 6 months
Standard practice
Sertraline starts at 25 or 50 mg a day and rises weekly in 25 to 50 mg steps, up to 200 mg. The label carries a boxed warning on suicidal thoughts up to age 24.
What happened in the studies
Beat placebo in 522 trials
In 522 trials, every antidepressant tested beat a dummy pill for depression.
¹ Lancet (London, England), 2018. The study is under The science, below.
Watch out
Sexual side effects are common. Never stop suddenly.
The science
- In detail
- The first-choice antidepressant for most men, on a prescription: sertraline, escitalopram or another SSRI. Sertraline starts at 50 mg a day, and the doctor steps it up if needed. Taken for at least six months once it works.
- Found
- 522 trials; all beat placebo. 522 trials: every one of 21 antidepressants beat placebo in major depression. Six trials pooled: SSRIs raised the risk of trouble reaching orgasm about threefold.
- Why
- SSRIs block the pump that clears serotonin back into the nerve, so more of it stays working. The mood effect builds over weeks as the brain adapts.
- Watch
- Boxed warning: antidepressants raised suicidal thoughts and behavior at ages 18 to 24 in short trials. Watch closely in the first months and after any dose change. Sexual side effects: delayed or failed ejaculation, lower drive, erectile trouble. Serotonin syndrome with other serotonin drugs. Bleeding risk with aspirin, NSAIDs or blood thinners. Low blood sodium: headache, confusion, unsteadiness. A heart-rhythm change (QTc) with other risk factors. Angle-closure glaucoma in narrow-angle eyes. Can trigger mania: tell the doctor about any highs. Never stop suddenly.

32Other antidepressantsBupropion, mirtazapine or an SNRI if the SSRI does not suitStrong evidence
Other antidepressants
For: The first pill did not suit
If the first antidepressant does not help or does not suit, others work on different chemistry.
What to do
- Tell the doctor what the first one did, good and bad.
- Sexual side effects: ask about bupropion. Poor sleep and appetite: ask about mirtazapine.
- Judge the new one at 4 weeks.
- How much
- The doctor’s choice
- When
- Once a day
- How long
- Judged at 4 weeks
Standard practice
Bupropion starts at 150 mg once a day, and the usual target is 300 mg. The dose rises slowly to keep seizure risk down. It can raise blood pressure.
What happened in the studies
Every one beat placebo
Every one of 21 antidepressants beat a dummy pill. Bupropion had fewer sexual side effects than SSRIs.
¹ International journal of molecular sciences, 2025. The study is under The science, below.
Watch out
Boxed warning: suicidal thoughts can rise at first, at younger ages. Bupropion is not for seizures, eating disorders, or stopping drink suddenly, and can raise blood pressure. Mirtazapine makes you sleepy and hungry.
The science
- In detail
- If an SSRI does not suit, the doctor switches or adds another. Bupropion when sexual side effects are the problem. Mirtazapine when sleep and appetite have gone. Or an SNRI such as venlafaxine or duloxetine. Bupropion starts at 150 mg and usually goes to 300 mg.
- Found
- Odds 1.37 to 2.13 over placebo. Across 21 antidepressants, every one beat placebo, some by odds of two to one. Pooled trials of bupropion: a lower risk of sexual side effects than the SSRIs.
- Why
- They work on different chemistry: bupropion on dopamine and norepinephrine, not serotonin, which is why it spares sex. Mirtazapine blocks receptors that also drive sleep and hunger.
- Watch
- All carry the same boxed warning on suicidal thoughts at younger ages. Bupropion is ruled out with a seizure disorder, a past eating disorder, an MAOI, or while stopping alcohol or benzodiazepines suddenly. Its seizure risk rises with the dose, and it can raise blood pressure. Mirtazapine: sleepiness and weight gain, so no driving until you know how it hits. Never combine two without the doctor.

- How long
- About 6 weeks
What happened in the studies
29%got better, against 10%
In 29 trials with 1,371 people, 29% responded, against 10% on a fake treatment. Responses began after about 13 sessions in the trials; a full clinic course is 20 to 30.
¹ Psychological medicine, 2014. The study is under The science, below.
Watch out
Scalp pain and headache at first. A small seizure risk, which is why it is done in a clinic.
The science
- In detail
- When two antidepressant courses have failed: ask for a course of high-frequency TMS over the left prefrontal cortex. Twenty to thirty sessions, weekday mornings, about six weeks. No anesthetic, no memory loss.
- Found
- 29% responded against 10% on fake. Twenty-nine trials against a fake treatment, 1,371 people. After about thirteen sessions, 29% responded and 19% saw their symptoms clear, against 10% and 5% on the fake. Across 113 trials compared together, high-frequency left TMS tripled the odds of a response.
- Why
- A coil on the scalp induces a current in the cortex beneath, and repeated pulses shift that region's excitability. The left prefrontal cortex runs under-active in depression, and the course drives it up.
- Watch
- Scalp pain and headache in the first sessions. Seizure risk is low under standard parameters, and is why it is done in a clinic.

Esketamine nasal spray
For: When pills did not help
A fast nasal spray for depression that tablets have not shifted. Clinic only.
What to do
- If two antidepressants have not worked, ask a psychiatrist.
- Twice a week in a clinic for 4 weeks, beside a daily tablet.
- Someone else drives you home.
- How much
- 56 or 84 mg
- When
- Twice a week, in clinic
- How long

What happened in the studies
Better by day 28
In a trial of 227 patients whom two antidepressants had failed, the spray plus a new tablet beat the tablet alone by day 28.
¹ The American journal of psychiatry, 2019. The study is under The science, below.
Watch out
Boxed warnings: sleepiness and feeling detached, slowed breathing, misuse, and suicidal thoughts at younger ages. Watched for 2 hours after each dose. Not with an aneurysm or past brain bleed.
The science
- In detail
- For depression that two antidepressants have not touched: a nasal spray given in a certified clinic, on its own or with a daily antidepressant. With acute suicidal thoughts, beside an antidepressant. Twice a week for four weeks, then less often.
- Found
- Better at day 28. A phase 3 trial, 227 patients who had failed at least two antidepressants: the spray plus a new antidepressant beat the new antidepressant alone at day 28, with gains seen earlier.
- Why
- Like ketamine, it blocks NMDA glutamate receptors, which sets off fast new connections between brain cells. That is why it works in days where tablets take weeks.
- Watch
- Boxed warnings: sedation, dissociation, slowed breathing, abuse and misuse, and suicidal thoughts at younger ages. Given only in a certified clinic, watched for at least 2 hours after each dose. Blood pressure rises. No driving until the next day after a night’s sleep. Ruled out with an aneurysm, an arteriovenous malformation, or past bleeding in the brain.
The study
Change in MADRS score with esketamine plus antidepressant was significantly greater than with antidepressant plus placebo at day 28
PMID 31109201 · Efficacy and Safety of Flexibly Dosed Esketamine Nasal Spray Combined With a Newly Initiated Oral Antidepressant in Treatment-Resistant Depression: A Randomized Double-Blind Active-Controlled Study. · The American journal of psychiatry, 2019
The sentence around it
Change in MADRS score with esketamine plus antidepressant was significantly greater than with antidepressant plus placebo at day 28 (difference of least square means=-4.0, SE=1.69, 95% CI=-7.31, -0.64); likewise, clinically meaningful improvement was observed in the esketamine plus antidepressant arm at earlier time points.…
The study supports the efficacy and safety of esketamine nasal spray as a rapidly acting antidepressant for patients with treatment-resistant depression.

35KetamineIn a clinic, when nothing else touched itModerate evidence
Ketamine
For: When pills did not help
A fast-acting treatment for depression that has not budged. Clinic only.
What to do
- If two courses of pills have not worked, ask a psychiatrist.
- Infusions in a clinic, with a plan to hold the gain.
- Never at home.
- How much
- A clinic series
- When
- Under a psychiatrist
- How long
- Days, then a plan
Standard practice
Ketamine’s label covers anesthesia only. Using it for depression is off-label, a psychiatrist’s call.
What happened in the studies
Works within a day
In 6 trials, it lifted depression from day 1 to day 7. In 79 real-world studies with 2,665 people, the effect held across repeat treatments.
¹ Psychiatry research, 2015. The study is under The science, below.
Watch out
Feeling detached and a blood pressure rise during the dose. Slowed breathing at high doses. Liver and bladder damage, and addiction, with repeated use.
The science
- In detail
- In a clinic, for a depression two courses of pills did not touch: an infusion series under a psychiatrist, and a plan for what holds the gain afterwards. Never at home, never as a weekend habit.
- Found
- effect from day 1 to day 7. Six trials against a placebo, 103 people: an antidepressant effect from day one to day seven. Seventy-nine real-world studies, 2,665 people: a substantial mean effect that holds across repeated treatments.
- Why
- It blocks NMDA receptors on the inhibitory interneurons first, which lifts the brake on the excitatory cells and sets off a glutamate surge that builds synapses within hours. That is why it works in a day where pills take a month, and why it wears off.
- Watch
- Dissociation and a blood-pressure rise during the dose, which is why it is observed. Slowed breathing with a high or fast dose. Liver and bile-duct injury with repeated use, so liver tests before a repeat course. Bladder damage and dependence with repeated unsupervised use.

36ECT, for the most severeECT, when the low is severe or dangerous nowStrong evidence
ECT, for the most severe
For: Severe and dangerous
A hospital treatment for the most severe depression. It works faster than tablets.
What to do
- A psychiatrist raises it when the low is severe or dangerous.
- A course of sessions in the hospital, under anesthetic.
- How much
- A course
- When
- In the hospital
- How long
- A few weeks
Standard practice
Considered for severe depression when a fast response is needed, as when it is life-threatening because a man is not eating or drinking, or when other treatments have not worked.
What happened in the studies
Beat medication
In 18 trials with 1,144 people, ECT did better than medication for depression.
¹ Lancet (London, England), 2003. The study is under The science, below.
Watch out
The risks of an anesthetic, and memory problems around the course.
The science
- In detail
- For severe depression that has not answered other treatment, or that is dangerous now, as when a man has stopped eating or drinking: a course of ECT under anesthetic, in the hospital.
- Found
- 18 trials; beat medication. Trials pooled: real ECT beat a fake version by a large margin. It beat medication in 18 trials of 1,144 people. It is an effective short-term treatment, probably more so than drugs.
- Why
- A brief, controlled seizure under full anesthetic resets the brain’s mood circuits faster than any tablet. That speed is why it is kept for the most dangerous lows.
- Watch
- The risks of a general anesthetic, and memory problems, mostly around the weeks of the course. Decided with a psychiatrist, with the risks laid out in full.
The study
Treatment with ECT was significantly more effective than pharmacotherapy (18 trials, 1144 participants, SES -0.80, 95% CI -1.29 to -0.29).

How you'll know it's working
- Track
- PHQ-8 score
- How to track
- Eight questions about the last two weeks, scored 0 to 24. Take it today, then every two weeks, same day of the week. Write the score in the diary.
- When to judge
- Training, sleep and the schedule show in the score inside a month. Review at two to four weeks; an antidepressant that will work usually shows it by four.
Why this happens
It has been weeks. You get through the day, but nothing lands: the gym, the food, the people, the things that used to be the point. You are not sad, exactly. You are flat.
Low mood often looks different on a man: a short fuse, irritable with people, more drinking, lost interest in sex, and trouble at work. Those count too.
The loop
Low mood makes you stop doing things, and stopping takes away the moments that lift it. Less effort, less reward, lower mood. Training, a schedule and people break the loop from the action side.
The body
Short sleep, a late body clock and nightly drinking all push mood down the next day. Morning light sets the clock, regular sleep steadies it, and cutting the drink gives the mornings back.
The thinking
The self-help course retrains the thinking: catch the low thought, test it, write a fairer one. When the low is deep or will not shift, the medicines act on serotonin, dopamine or glutamate. They go through a doctor, and they work best on top of the rest.
Week by week
Why this order
The moves you do yourself and the self-help course first, medicine by preference or when the low is heavier.
| When | Add | Why then |
|---|---|---|
| Today | The PHQ-8. A score of 16 or more, or weeks of not eating or working: the doctor this week. | Safety and severity decide where a man starts. Everything else waits for them. |
| Week 0 | Training most days, lifting twice a week. One enjoyable and one avoided thing scheduled every day. Fixed wake time, morning light, the drinks counted and cut. Tell one person. | The free moves with the strongest trials. They start together because they feed each other. |
| Week 1 | Three good things each night. Start the six-week self-help course: this week, log what you do each hour and rate your mood. | The course is built on the methods tested in trials, and you can start it tonight, with no waiting list. |
| Week 2 | One supplement if you want one: saffron, curcumin, EPA-rich fish oil, St John’s wort or SAMe. Never St John’s wort or SAMe with an antidepressant. | One at a time, so a change in the score can be credited. |
| Weeks 2 to 4, the review | This score against the first. Down: keep going. Flat or up, or still 16 or more: the doctor, with the diary, to talk about an antidepressant. | The UK’s NICE guideline reviews treatment at two to four weeks. A low that is not moving is a reason to add, not to wait. |
| On an antidepressant | A review at one week if you are 25 or under, two weeks otherwise. Give it four weeks. Once it works, at least six months. Never stop suddenly. | The early weeks carry the suicidality warning up to age 24, and stopping fast brings withdrawal. |
| After two failed antidepressants | A psychiatrist: TMS, esketamine, ketamine or, for the most severe, ECT. | The clinic treatments are built for the depression tablets have not shifted. |
| Any week | Worry and a racing mind louder than the low: run Stay calm beside this page. Sleep the main problem: Deeper sleep. | Anxiety and poor sleep ride with low mood, and each has its own page. |
| Once it has lifted | Eight weeks of mindfulness on your own. Keep the training, the schedule, the log and the people. | Relapse is the risk after a first low. The daily practice and the habits hold the gain. |
What to track
One number: the PHQ-8 score, every two weeks. Under 5 is the goal. Five to 9 is mild, 10 to 14 moderate, 15 to 19 moderately severe, 20 and up severe. Read the trend over a month, not one score.
The diary covers a week. Log the days you trained, the good things you scheduled and did, hours slept, drinks, and one line on mood. If the score has not moved by the review, the order above says what to do next.
The diary, one week
| Day | Trained today? | Scheduled thing done? | Hours slept | Drinks | Mood, 1 to 10 |
|---|---|---|---|---|---|
| Mon | |||||
| Tue | |||||
| Wed | |||||
| Thu | |||||
| Fri | |||||
| Sat | |||||
| Sun |
- The PHQ-9 score every two weeks: judged at the review, two to four weeks in, then monthly. The last question is the one to watch.
- Days trained out of seven, and scheduled things done: the columns that move first.
- Hours slept and drinks a week: the two that set the next morning.
- On an antidepressant: mood, sleep, sex and any new dark thoughts, one line a week. A change is a call to the doctor, not a column to watch.
- A thyroid blood test if tiredness, feeling cold and weight gain came with the low. It is a check, not a move.

Words on this page
- Standardized mean difference (SMD)
- A way to put results measured on different scales onto one scale. About 0.2 is a small effect, 0.5 medium and 0.8 large.
- 95% CI
- The range the true result probably sits in. The narrower the range, the surer the result.
- Meta-analysis
- Many studies pooled into one result.
- RCT
- A randomized controlled trial: people put by chance on the treatment or on a dummy version.
- PMID
- A study's ID number in PubMed, the public index of medical research.
- Strong, moderate and early evidence
- Strong: many human trials agree. Moderate: human studies point the same way. Early: the first small human studies.
- PHQ-8
- A nine-question score for depression, from 0 to 27. Ten or more is the usual line for a doctor’s look.
- Behavioral activation
- A method that schedules rewarding and avoided activities back into the week, so mood follows action.
- CBT
- Cognitive behavioral therapy: the method tested in the trials behind the self-help course. It tests the thoughts and habits keeping a low going.
- SSRI
- Selective serotonin reuptake inhibitor: the first-choice antidepressant class, sertraline and escitalopram among them.
- SNRI
- An antidepressant that acts on serotonin and norepinephrine, venlafaxine and duloxetine among them.
- Boxed warning
- The strongest warning on a US drug label, printed in a box at the top.
- TMS
- Transcranial magnetic stimulation: magnetic pulses to the front of the brain, in a clinic, for depression tablets have not shifted.
- ECT
- Electroconvulsive therapy: a brief controlled seizure under anesthetic, for the most severe depression.
- Relapse
- The low coming back after it has lifted.
The score tonight, one person told, and one good thing planned for tomorrow. Training, the schedule, sleep and people do the heavy lifting, and the self-help course starts this week. Everything else is added one at a time and judged on the score.
When to see a doctor
- The low is severe: you have stopped eating or drinking, or cannot get out of bed or to work. See a doctor urgently.
- New or worse low mood, agitation or dark thoughts after starting an antidepressant or changing the dose: call the doctor right away.
- Low mood, no interest or no enjoyment for most of the day, every day, for more than 2 weeks: see your doctor.
- Low mood that started on finasteride, or after coming off anabolic steroids.
- Tired, cold, gaining weight and low together: ask for a thyroid blood test.
- Ask your doctor about blood tests that rule out look-alikes: thyroid, vitamin B12, anemia and vitamin D.
- Low mood with lost sex drive, erection trouble and no energy: ask about a testosterone test.
- Loud snoring, gasping in sleep and tired all day: ask about sleep apnea, which brings low mood with it.
- Using cannabis most days: it can flatten motivation and mood. Tell the doctor.
- Ever had days feeling wired and needing little sleep: tell the doctor before light therapy, SAMe, St John’s wort or an antidepressant.
I am not a doctor. This is not medical advice. This is self-education. Every dose on this page is the one a trial or a label used, quoted from the source. It is not a prescription. Talk to a physician who has your history before anything in the With a doctor group.
