Social anxiety

Also called Shyness, Social awkwardness, Social phobia, Confidence around people

3 min plan59 min with every studyUpdated Sep 27, 2026

Who it's forA man who dreads talking to people, avoids social situations, and replays conversations afterwards. Or who wants to be at ease and confident with people.

Tonight

  1. Take the SPIN and write the score down
  2. List ten situations you avoid, easiest first
  3. Pick one for tomorrow, and write what you think will happen

The plan

PREDICT THE WORSTWATCH FROM INSIDELEAN ON PROPSREPLAY ITEXPOSURE LADDEREASIEST FIRST
Figure 1. Social fear is a loop: predict the worst, watch yourself from the inside, lean on props, then replay it. The exposure ladder breaks it: the situations you avoid, easiest first, each prediction tested.

The SPIN social anxiety scoreThe number to track: the SPIN score, every two weeksCheck

The SPIN social anxiety score

For: Knowing where you stand

A 17-question score that shows how strong the social fear is, and whether it is dropping.

How to check

  1. Answer the 17 questions about the last week.
  2. Write the score down.
  3. Repeat every 2 weeks and watch the trend.

Standard practice

Screen with the three-item Mini-SPIN: 6 or more means a full assessment. Track progress with a standard scale such as the SPIN or the LSAS.

Reading the number

Tracks the trend

A score of 19 or more separated social anxiety from no social anxiety. The 3-question short form, at 6 or more, caught it 88.7% of the time.

¹ The British journal of psychiatry, 2000. The study is under The science, below.

The science
In detail
Seventeen questions on fear, avoidance and body symptoms in social situations over the last week. Take it today, then every two weeks, and bring them to the doctor if you see one. A clinic may use the longer 24-item LSAS instead.
Found
A score of 19 is the line. The 17-item self-rated scale held up on reliability and validity across three trials. A score of 19 separated social anxiety from controls, and the score tracked change over time. Its three-item short form, at 6 or more, caught the condition 88.7% of the time.
Why
Social fear hides its own progress: every bad moment feels like proof nothing has changed. A score on paper every two weeks shows the trend, and whether the moves are working.
How it is done
Seventeen questions; Every two weeks; Today, then every 2 weeks

Standard practice

The method tested in trials builds in graded exposure to feared social situations, practiced again and again, and experiments that test the negative beliefs.

What happened in the studies

Large drop in social anxiety

Across 101 trials with 13,164 people, exposure and social skills training cut social anxiety a lot.

¹ The lancet. Psychiatry, 2014. The study is under The science, below.

The science
In detail
List the situations you avoid, easiest first: a question to a shop worker, a call, a comment in a meeting, a party. Before each, write what you expect to happen. Go in, stay, then write what did happen. Several a week, in different places.
Found
Effect 0.86 against waitlist. A review of 101 trials and 13,164 people: exposure and social skills training cut social anxiety against a waitlist by a large effect. Exposure practice today is built on learning that the feared outcome does not come. That cuts fear and avoidance for the long term.
Why
Social fear is a prediction: they will think I am boring, I will go red, I will freeze. Avoiding keeps it untested, so it stays true by default. Each time you go in and it does not happen, the prediction weakens.

Standard practice

The method tested in trials uses experiments to show how self-focused attention and safety-seeking behaviors make the anxiety worse.

What happened in the studies

Fewer symptoms

Pooled across 19 studies, cutting the props reduced symptoms. Added to a course, it gave extra benefit.

¹ Clinical psychology review, 2026. The study is under The science, below.

The science
In detail
List what you do to get through: the rehearsed lines, the drink first, the phone in your hand, no eye contact, the hood up, leaving early. In the next situation, drop one. Then the next one.
Found
Symptoms down, d 0.43. Nineteen studies pooled: cutting safety behaviors reduced them and reduced symptoms against the comparison group, by a small effect. In a group course for social fear, adding the instruction to drop them gave a clear extra benefit.
Why
A prop lets you get through without ever finding out you could manage alone. You leave thinking it went fine because you rehearsed, so the fear stays where it was. Drop it and the situation reports back on you.

The studies

relative to control, safety behavior reduction led to significantly greater reductions in safety behaviors (d = 0.52, 95% CI [0.36, 0.68]) and symptoms (d = 0.43, 95% CI [0.27, 0.59])

Standard practice

The method tested in trials includes systematic training in putting attention outward.

What happened in the studies

Less fear of blushing and shaking

In a trial of 65 people, training attention outward beat relaxation for the fear of blushing and shaking at one year.

¹ Behaviour research and therapy, 2006. The study is under The science, below.

The science
In detail
In a conversation, put your attention on the other person: their face, their words, what they are telling you. When you catch yourself checking how you look or sound, notice it and turn back out. Practice it in easy chats first.
Found
Effect size 3 at one year. A trial of 65 people on the fear of blushing and shaking. Attention training beat relaxation, each added to the same course, at one year, with effect sizes of 3. Across 226 effect sizes, watching yourself from outside tracked with social anxiety.
Why
You cannot watch yourself and watch them at the same time. The inside view is built from how you feel, not what they see, so it invents an audience that is not there. Turning attention out fixes both.

¹ Journal of personality and social psychology, 2011. The study is under The science, below.

The science
In detail
Join one thing that meets weekly with the same people: a team, a class, a club, a gym group, a volunteering shift. Turn up every week, even when you say little at first.
Found
More contact, more liking. Two experiments with people meeting for the first time and talking for different lengths of time: the more they talked, the more they liked each other. Feeling understood, getting comfortable and knowing each other carried the effect.
Why
Liking grows with contact. Repetition builds the comfort that makes talking easier, and it turns strangers into people who expect you. A standing weekly room is exposure that never has to be scheduled.

The study

The more participants interacted, the more attracted they were to each other.

PMID 21381850 · Familiarity does indeed promote attraction in live interaction. · Journal of personality and social psychology, 2011

…Findings strongly supported the "familiarity leads to attraction" hypothesis: The more participants interacted, the more attracted they were to each other.

Mediation analyses identified three processes that contribute to this effect: perceived responsiveness, increased comfort and satisfaction during interaction, and perceived knowledge.

The science
In detail
Once a day, start a short chat with someone you do not know. The barista, the man next to you on the train, someone in line at the gym. A question or a comment is enough.
Found
Better journeys when they talked. In experiments on trains and buses, people told to talk to a stranger had the more positive journey, though others predicted the opposite. People underestimated how keen strangers were to talk.
Why
The fear says strangers do not want to be bothered. The experiments say they mostly do. Short daily chats are low-stakes exposure, and each one corrects the prediction a little.

¹ BMJ open sport & exercise medicine, 2026. The study is under The science, below.

Watch out

Build up gradually to avoid injury.

The science
In detail
Lift two or three times a week, and add walking or jogging on other days. Training in a group or a class adds a weekly room full of the same faces.
Found
Medium drop in anxiety. A review of 30 trials: for anxiety disorders, resistance training came out most effective, by a medium effect. Across 45 studies, lifting, aerobic work and walking or jogging all lowered anxiety.
Why
Training burns off the stress hormones that drive the pounding heart and shaking hands, and it teaches the body that a racing pulse is not danger. It also lifts the mood that social fear drags down.

Standard practice

Drinking or drug use is often an attempt to calm social anxiety, and it does not rule out treatment. Harmful drinking gets a brief treatment; dependent drinking, a specialist service.

What happened in the studies

Better mental health

In 1,383 people followed for a year, every step down in drinking came with better mental health. A bigger cut meant a bigger gain.

¹ Alcoholism, clinical and experimental research, 2017. The study is under The science, below.

Watch out

If you drink every day and shake without it, stopping suddenly is a medical danger. See a doctor first.

The science
In detail
Count the drinks for a week, and mark the ones you had to get through a social event. Then cut a level, and go to one event a week with no drink first. If you drink to face every event, tell your doctor.
Found
1,383 people, one year. 1,383 people, one year: every step down the risk-drinking ladder came with fewer alcohol consequences and better mental health. The gain held a year on.
Why
Drink before an event is a safety behavior in a glass. It gets you through without learning you could do it, and the next day the anxiety rebounds. Cutting it lets the fear be tested sober.
Watch
Physical dependence, where cutting abruptly is a medical event and not a plan

¹ Frontiers in psychology, 2024. The study is under The science, below.

Watch out

Too much can cause a racing heart, shaking or panic.

The science
In detail
Keep to your normal coffee and never extra before a social event. Stay under 400 mg across the day: past that, the rise in anxiety was largest.
Found
Anxiety up most above 400 mg. Fourteen studies, 546 people: caffeine raised anxiety overall, moderately at low doses and a lot at high doses. The rise was largest above 400 mg a day.
Why
Caffeine pushes adrenaline up: the pounding heart, the shaking hand, the sweat. Those are the exact signs a socially anxious man fears people will see. Extra before an event adds to them.
Watch
More than your usual can bring on a racing heart, shaking and, in some, panic.

¹ Sleep medicine reviews, 2021. The study is under The science, below.

Watch out

Sleeping 9 hours or more and still tired is a question for a doctor.

The science
In detail
Seven to eight hours, the same wake time seven days a week. A short night turns the alarm up the next day, and the replay runs louder at 3 am.
Found
g = 0.63 on depression. Sixty-five trials, 8,608 people: when a treatment improved sleep, depression fell by a medium effect. Anxiety and rumination fell with it. The more sleep improved, the more mood did.
Why
A short night raises the amygdala's reactivity and cuts the prefrontal brake on it the next day. Regular sleep is the one move here that every other move depends on.
Watch
Nine hours or more a night that does not refresh is a doctor's question, not a reason to sleep in.

The study

Improving sleep led to a significant medium-sized effect on composite mental health (g+ = -0.53), depression (g+ = -0.63), anxiety (g+ = -0.51), and rumination (g+ = -0.49)

¹ Journal of affective disorders, 2023. The study is under The science, below.

The science
In detail
Film yourself in a short conversation or a two-minute talk. Before you watch, write in detail what you expect to see: how red, how shaky, how awkward. Then watch it as if it were a stranger, and compare.
Found
92% to 96% looked calmer than predicted. In two trials, 92% online and 96% face to face thought they looked less anxious than predicted. Social anxiety fell after the video. Writing the prediction first raised how people rated their own performance.
Why
The picture in your head is built from how you felt, and it is far worse than how you looked. Writing the prediction pins it down, so the footage has something to collide with.

The studies

92 % of participants in iCT-SAD, and 96 % in CT-SAD thought they looked less anxious compared to their predictions after viewing the videos.

When the replay of a conversation starts, name it: that is the post-mortem. Remind yourself you were liked more than you think. Then let the thought sit there without arguing with it, and go back to what you are doing. Do the same with the dread before an event.
Found
Liked more than they knew. Across several studies, people underestimated how much the person they talked to liked them, and the gap lasted for months. A trial of 52 people: banning the pre-event replay cut how often it came and how much it hurt.
Why
The replay feels like learning, but it only replays the parts that felt bad, from the inside. It keeps the fear fed between events. Naming it and leaving it alone starves it.

Our studies suggest that after people have conversations, they are liked more than they know.

Six breaths a minute, daily and before you go in

For: Stress, anxiety

Slow breathing switches your body out of fight-or-flight. It is the fastest way to calm down.

What to do

  1. Breathe in for 4 seconds and out for 6.
  2. Do it 10 to 20 minutes a day, and any time you feel wound up.

How long

What happened in the studies

Large drop in stress

In 24 studies, slow breathing cut stress and anxiety a lot. Done at home, it also lowered depression.

¹ Psychological medicine, 2017. The study is under The science, below.

Watch out

If you feel dizzy or tingly, breathe more gently.

The science
In detail
Six breaths a minute: in for four, out for six. Ten to twenty minutes a day, and any time you are wound up. A strap or a wearable that shows the heart rate while you breathe closes the loop. The trials found under twenty minutes a day worked best.
Found
g = 0.83 against control. Twenty-four studies, 484 people: a large drop in self-reported stress and anxiety against control. Ten studies pooled, run at home: depression down by a medium effect and HRV up by the same.
Why
Breathing near six a minute puts the breath in phase with the blood-pressure reflex, and the heart swings wide with each breath. That swing is the vagal brake being used. Used daily, it stays stronger between sessions.
Watch
Dizziness or tingling means you are blowing off too much carbon dioxide. Breathe lighter, not deeper.

The studies

The between-groups analysis comparing biofeedback to a control condition yielded Hedges' g = 0.83.

PMID 28478782 · The effect of heart rate variability biofeedback training on stress and anxiety: a meta-analysis. · Psychological medicine, 2017

Meta-analysis showed a medium effect size of HRV-B for improving both depression (g = - 0.41 [- 0.049, - 0.772], p = 0.026, I 2 = 72.623%, k = 10) and HRV (g = 0.443, [0.718, 0.167], p = 0.002, I 2 = 56.81%, k = 10) compared to control conditions.

¹ Journal of psychiatric research, 2024. The study is under The science, below.

Watch out

Some people feel nauseous or dizzy in a headset.

The science
In detail
A self-guided virtual-reality app on your phone in a simple headset: feared scenes from easy to hard, a room of faces, an interview, a date, a talk. Work up the scenes over six weeks, then do the same things for real.
Found
Effect size 1.8. A trial of 126 people ran six weeks. The self-guided headset app cut the social anxiety score by 35.7 points, against 2.2 on the waitlist, an effect size of 1.8. Across 22 studies, headset practice kept its gain over long follow-up.
Why
It lets you enter the feared scene on demand, repeat it and turn up the pressure step by step. That makes the first real attempts smaller steps.
Watch
Some people feel nauseous or dizzy in a headset. Keep the real-world steps going too.

The studies

The reduction in LSAS scores was significantly greater in the active group compared to the waitlist delayed treatment group (active group mean = -35.7 [SD = 24.0]; waitlist group: mean = - 2.2 [SD = 13.5]; p < 0.001), for an effect size of 1.8.

Early research, clinic only. For social anxiety that the self-help course and two medicines have not moved, ask a psychiatrist about trials. The first trial gave one infusion of 0.5 mg per kg over 40 minutes, under observation.
Found
33% responded against none. A trial of 18 people: one ketamine infusion cut social anxiety more than saline. 33% responded in two weeks, against none. A dose study of 12 people: calmer within an hour, for up to a week.
Why
Ketamine blocks NMDA glutamate receptors, which sets off fast new connections between brain cells. That may loosen fear learning quickly, where tablets take weeks.
Watch
A controlled drug: Schedule III in the US, Class B in the UK. Its label covers anesthesia only, so this use is off-label. The science
In detail
Early research. In the trial, one 600 mg dose of cannabidiol 90 minutes before a speech test, against a dummy dose. In the US it is a prescription seizure medicine, and not lawful in food or supplements.
Found
Speech anxiety down, one dose. A trial of 24 people: one dose of cannabidiol before a public-speaking test cut anxiety, thinking trouble and discomfort during the speech. A review of the human studies found single doses lowered anxiety without sedation.
Why
Cannabidiol acts on serotonin 1A receptors and the body’s own cannabinoid system, both involved in fear. It does not carry the high of THC.
Watch
The prescription label warns of liver injury, with liver blood tests before starting, and higher risk beside valproate. It warns of sleepiness and sedation: no driving until you know how it hits. It also warns of suicidal thoughts, and of allergic reactions. It interacts with other medicines.

The science
In detail
If you would rather take a medicine, or the self-help course has only half worked: on a prescription. The label starts at 25 mg a day for social anxiety, then 50 mg after a week, rising in weekly steps if needed, up to 200 mg. Judged at 10 to 12 weeks.

A 20-week trial of 204 people, started on 50 mg: 53% responded on sertraline against 29% on placebo. It beat placebo on every main measure. Across 101 trials, SSRIs and SNRIs beat placebo.
Why
SSRIs block the pump that clears serotonin back into the nerve, so more of it stays working. The calming effect builds over weeks as the brain adapts, which is why it is judged at three months.
Watch
Boxed warning: antidepressants raised suicidal thoughts and behavior in young adults in short trials, 5 more per 1,000 at ages 18 to 24. Watch closely in the first months and after any dose change. Never with an MAOI, or within 14 days of one, and never with pimozide. Serotonin syndrome with other serotonin drugs, triptans and tramadol among them. Bleeding risk with aspirin, NSAIDs or blood thinners. It raises the level of some drugs, warfarin and CYP2D6 drugs among them. Can trigger mania: tell the doctor about any highs. Seizures. Angle-closure glaucoma in narrow-angle eyes. Low blood sodium: headache, confusion, unsteadiness. A heart-rhythm change (QTc) with other risk factors. Sexual side effects: delayed or failed ejaculation, lower drive, erectile trouble. It can cause early jitteriness and poor sleep. Never stop suddenly.

Sexual side effects can last after stopping. Never stop suddenly.

The science
In detail
The other first-choice SSRI for social anxiety in the UK's NICE guideline, on a prescription. The UK label dose is 10 mg once a day. The doctor can lower it to 5 mg or raise it to 20 mg. Relief usually takes 2 to 4 weeks. The course runs 12 weeks.
Found
A 12-week trial of 358 people, at 10 to 20 mg: 54% responded on escitalopram against 39% on placebo. Three trials, 1,598 people: every dose beat placebo on the social anxiety scale.
Why
It is the most selective of the SSRIs: it blocks the serotonin pump and little else. The effect builds over weeks as the brain adapts.
Watch
Boxed warning: antidepressants raised suicidal thoughts and behavior at younger ages; the UK label names under 25. Watch closely early and after dose changes. Never with an MAOI, linezolid, intravenous methylene blue or moclobemide, and never with pimozide. Ruled out with a known long QT or with other drugs that lengthen the QT. Heart-rhythm risk rises with the dose, low potassium or magnesium, and heart disease: an ECG first if you have one. Serotonin syndrome with triptans, tramadol, tryptophan or St John’s wort. Low blood sodium, more broken bones, and it weakens tamoxifen.

The science
In detail
If sertraline or escitalopram did not suit: another SSRI. UK and US: prescription, licensed for social anxiety. The label starts and keeps it at 20 mg a day. It needs a slow, planned stop.
Found
A 12-week trial: the social anxiety score fell 39.1% on paroxetine against 17.4% on placebo. A 12-week trial of 440 people: paroxetine beat placebo from week 3 to week 12 on the main scales.
Why
It blocks the serotonin pump like the other SSRIs. It also blocks a second receptor, which brings drowsiness and a strong withdrawal, so it usually comes after the first two.
Watch
Boxed warning: antidepressants raised suicidal thoughts and behavior at younger ages, 5 more per 1,000 at 18 to 24. Watch closely early and after dose changes. Angle-closure glaucoma in narrow-angle eyes. Sexual side effects, drowsiness and sweating are common. The withdrawal syndrome is among the strongest of these drugs: stop slowly, never suddenly. Low blood sodium. A higher risk of bone fracture. It weakens tamoxifen: on tamoxifen, another antidepressant is used.

Mean Liebowitz Social Anxiety Scale total scores were reduced by 39.1%

PMID 9728642 · Paroxetine treatment of generalized social phobia (social anxiety disorder): a randomized controlled trial. · JAMA, 1998

The sentence around it

Mean Liebowitz Social Anxiety Scale total scores were reduced by 39.1% (the mean baseline score of 78.0 declined by a mean of 30.5 points at follow-up) in the paroxetine group compared with 17.4% (the mean baseline score of 83.5 declined 14.5 points at follow-up) in the placebo group, a difference of 21.7% (95% CI, 8.7%-34.7%) favoring paroxetine.…

Paroxetine treatment was significantly superior to placebo at weeks 3 through 12 on the Liebowitz Social Anxiety Scale, the Clinical Global Impression-Severity of Illness scale, and the Social Phobia Inventory, and at weeks 4 through 12 for response (P < .05 for all).

23Venlafaxine, an SNRIVenlafaxine XR 75 mg a day, if the SSRIs did not suitStrong evidence

Venlafaxine, an SNRI

For: The SSRIs did not suit

A different class of antidepressant, licensed for social anxiety in the UK and the US. Blood pressure gets checked.

What to do

  1. Ask the doctor about an SNRI if an SSRI did not help.
  2. 75 mg once a day with food.
  3. Get your blood pressure checked.
  4. Give it 10 to 12 weeks. Never stop suddenly.
How much
75 mg
When
Once a day, with food
How long
Judged at 10 to 12 weeks

Standard practice

Prescription only, licensed for social anxiety in the UK and the US at 75 mg once a day, with food. Blood pressure is controlled first and checked through treatment. When stopping, the dose is tapered.

What happened in the studies

58.6%against 36.1%

In a 12-week trial of 440 people, 58.6% improved on venlafaxine against 36.1% on a dummy pill.

¹ Archives of general psychiatry, 2005. The study is under The science, below.

Watch out

Boxed warning: at 24 and under it can raise suicidal thoughts at first. Never with an MAOI. Raises blood pressure, so it gets checked. Bleeding, low sodium, seizures, manic highs, an eye-pressure emergency. Sexual side effects. More dangerous in an overdose. Strong withdrawal: taper slowly.

The science
In detail
If the SSRIs did not suit: venlafaxine extended-release, on a prescription, licensed for social anxiety in the UK and the US. The label dose is 75 mg once a day with food. Blood pressure is checked.
Found
A 12-week trial of 440 people: 58.6% responded on venlafaxine and 62.5% on paroxetine, against 36.1% on placebo. A second 12-week trial: 44% responded against 30%, and 20% against 7% reached remission.
Why
An SNRI blocks the pumps for both serotonin and norepinephrine. Some men respond to it when an SSRI did not.
Watch
Boxed warning: antidepressants raised suicidal thoughts and behavior at younger ages, 5 more per 1,000 at 18 to 24. Watch closely early and after dose changes. Never with an MAOI or within 14 days of one, linezolid or intravenous methylene blue among them. Serotonin syndrome with other serotonin drugs. Raises blood pressure: it is controlled first and checked regularly. Bleeding risk with aspirin, NSAIDs or blood thinners. Angle-closure glaucoma. Can trigger mania: screened for bipolar first. Seizures. Low blood sodium. Rare lung inflammation: report a new cough or breathlessness. Sexual side effects. More dangerous than the SSRIs in an overdose. A strong withdrawal: taper, never stop suddenly.

The studies

Week 12 response rates were significantly greater for the venlafaxine ER and paroxetine groups (58.6% and 62.5%, respectively) vs the placebo group (36.1%) (P < .001 for both).

PMID 15699296 · Venlafaxine extended release vs placebo and paroxetine in social anxiety disorder. · Archives of general psychiatry, 2005

Week 12 response and remission (LSAS score < or = 30) rates were significantly greater in the venlafaxine ER group than in the placebo group (response: 44% vs. 30%, respectively, p = .018; remission: 20% vs. 7%, respectively, p < .01).

24Propranolol, for a one-off performancePerformance only, through a doctor: 40 mg ahead of the eventEarly evidence

Propranolol, for a one-off performance

For: A speech or presentation

A prescription pill that blocks adrenaline for one big performance. Your heart stays steady and your hands stop shaking.

What to do

  1. Use it only for a one-off performance, through a doctor.
  2. Try a test dose on a normal day first.
  3. On the day, take it ahead of the event, as the doctor says.
How much
40 mg
When
Ahead of the event
How long
On the day only

Standard practice

Prescription only. Beta-blockers control the shaking, sweating and fast heart, and are the usual choice for the performance kind of social anxiety. The UK label covers situational anxiety; the US label does not list anxiety, so there it is off-label.

What happened in the studies

Steadier hands, better performance

In two trials, it removed the physical signs of stage fright, and critics rated the performance higher.

¹ The American journal of medicine, 1982. The study is under The science, below.

Watch out

Never with asthma, chronic bronchitis or emphysema: it can set off an attack. Not with a slow heart, heart block, low blood pressure or heart failure. Fit with a low resting pulse? Tell the doctor. Tell them about any antidepressant or heart drug. An epinephrine pen may work less well. It can hide low blood sugar, and alcohol raises its level. Dizziness, tiredness, cold hands, nightmares. Stop at a rash or swelling. Dangerous in overdose. Taken daily, never stop suddenly.

The science
In detail
For one thing only: a speech, a presentation, an exam in front of people. Not for everyday social fear, which the moves above treat. Only through a doctor. The usual anxiety dose is 40 mg, taken ahead of the event. Test it on an ordinary day first. The full routine is on Stay calm.
Found
Stage-fright signs removed. Two trials: blocking beta receptors removed the physical signs of stage fright, dry mouth included. Critics scored the performance higher, and it did this without sedation.
Why
Performance nerves are adrenaline hitting beta receptors: pounding heart, shaking hands, dry mouth, a tight voice. Propranolol blocks those receptors, so the body stays quiet without sedating the mind.
Watch
Never with asthma, and not with chronic bronchitis or emphysema: it can bring on an attack. Not with a slow heart rate, heart block, low blood pressure or worsening heart failure. A trained man with a low resting pulse raises it with the doctor. Tell the doctor about any antidepressant: fluoxetine, paroxetine and fluvoxamine raise its level. With some calcium-channel blockers it can slow the heart badly and cause heart failure. An epinephrine pen may work less well. It hides the warning signs of low blood sugar, and alcohol raises its level. Dizziness, tiredness, cold hands, nightmares. Stop at a rash or swelling. Dangerous in overdose. Taken daily, never stop it suddenly. Banned in competition in some precision sports.

The study

Beta blockade eliminates the physical impediments to performance caused by stage fright and even eliminates the dry mouth so frequently encountered.

PMID 6120650 · Effect of beta blockade and beta stimulation on stage fright. · The American journal of medicine, 1982

This effect is achieved without tranquilization.

How you'll know it's working

Track
SPIN score
How to track
Seventeen questions about the last week. Take it today, then every two weeks, same day of the week. Also count the feared situations you walked into each week.
When to judge
Exposure and dropped props move the score within weeks. The self-help course runs 8 to 9 weeks; an SSRI is judged at 10 to 12 weeks.

Why this happens

You dread the party, the meeting, the call. You rehearse what to say, get through it, then lie awake replaying every word and every pause.

It is not shyness you will grow out of. It is a loop: predict the worst, watch yourself from the inside, lean on props, then replay it. It is common, and it is very treatable.

The prediction

Before a social moment, the mind predicts the worst: they will see me go red, I will have nothing to say. Avoiding the moment keeps the prediction untested, so it stays true by default.

The inside view

In the moment, attention turns inward to how you look and sound, and props take over: rehearsed lines, a drink, the phone. The picture in your head is worse than what they see.

The replay

Afterwards, the post-mortem replays only the bad parts, from the inside. It feeds the next prediction. The self-help course, tested predictions and the medicines break the loop at each point.

WhenAddWhy then
TodayThe SPIN. Stopped leaving the house, or drinking to face every event: your doctor this week.Safety and severity decide where a man starts. Everything else waits for them.
Week 0The avoid list, easiest first. One situation several times a week, prediction written before. One prop dropped each time. Attention on them, not you. One stranger a day. Join one weekly group. These are the working parts of the tested methods, and they start working at once.
Week 1Start the eight-week self-help course, one module a week, or a self-help book or app. Film yourself once and compare. The course puts the moves in order, and the online version matched face to face in trials.
Week 2Training two or three times a week. Drinks counted, none before one event a week. Coffee at your usual.Drink before an event is a prop, and extra caffeine adds the shaking you fear.
Every 2 weeksThis SPIN against the first. Down: keep going. Flat or rising after the full course: your doctor, and ask about an SSRI.A score that will not move is the line to bring in a doctor.
On an SSRIUnder 30, a check within a week and weekly for the first month. Then every 2 to 4 weeks for 3 months. Judge it at 10 to 12 weeks. Partly working: add the self-help course. Working: at least 6 more months, then taper. The first weeks carry the suicidality warning, and the full effect takes weeks to build.
No response to the SSRINo response at 10 to 12 weeks, or side effects you cannot live with: back to the doctor. The UK's NICE guideline has a next SSRI or a related medicine, then a further option.A first medicine that fails is a reason to move to the next, not to stop.
A one-off speechShaking and a pounding heart that ruin a single performance: ask a doctor about propranolol, tested on an ordinary day first. The full routine is on Stay calm.It calms the body for one event. The everyday fear is what the rest of the page treats.
Any weekLow, flat, no interest in things for weeks: run Low mood beside this page, and tell the doctor which came first.Depression often rides with social anxiety, and which came first decides what is treated first.

What to track

One number: the SPIN score, every two weeks. A score of 19 or more is the line the scale uses for social anxiety. Read the trend over a month, not one score.

The diary is a week: feared situations you walked into, props dropped, strangers talked to, drinks before events, and one line on the dread. Where the score has not moved by the review, the order above says what to do next.

  • The SPIN score every two weeks: judged at each review, and at the end of the course or 10 to 12 weeks on an SSRI.
  • Feared situations entered per week: the column that moves first.
  • Drinks before events: a prop to count down to zero.
  • On an SSRI: mood, sleep, sex and any new dark thoughts, one line a week. A change is a call to the doctor, not a column to watch.
  • A thyroid blood test if nervousness came with weight loss, a racing heart and feeling hot. It is a check, not a move.

Deep dives

The studies

Every study behind this page, newest first.

Words on this page

Standardized mean difference (SMD)
A way to put results measured on different scales onto one scale. About 0.2 is a small effect, 0.5 medium and 0.8 large.
95% CI
The range the true result probably sits in. The narrower the range, the surer the result.
Meta-analysis
Many studies pooled into one result.
RCT
A randomized controlled trial: people put by chance on the treatment or on a dummy version.
PMID
A study's ID number in PubMed, the public index of medical research.
Strong, moderate and early evidence
Strong: many human trials agree. Moderate: human studies point the same way. Early: the first small human studies.
SPIN
The Social Phobia Inventory: 17 questions on social fear, avoidance and body symptoms. A score of 19 or more is the usual line.
Mini-SPIN
The three-question short form of the SPIN, used to screen. Six or more means a full assessment.
LSAS
A 24-item social anxiety scale used in clinics and trials.
Safety behavior
A prop used to get through a feared situation: rehearsed lines, a drink first, avoiding eye contact. It keeps the fear in place.
Exposure
Walking into a feared situation on purpose, and staying, so the prediction gets tested.
Post-event processing
The replay after a social event, going over what went wrong. The post-mortem.
SSRI
Selective serotonin reuptake inhibitor: the first-choice medicine class for social anxiety, sertraline and escitalopram among them.
Boxed warning
The strongest warning on a US drug label, printed in a box at the top.
Beta-blocker
A drug that blocks adrenaline at the heart and other organs. Propranolol is one.

The score tonight, the avoid list written, and the first situation picked for tomorrow. Going in, dropping the props and attention out do the heavy lifting, and the self-help course starts this week. Everything else is added one at a time and judged on the score.

When to see a doctor
  • New or worse anxiety, agitation or dark thoughts after starting an SSRI or changing the dose: call the doctor right away.
  • You have stopped leaving the house, or going to work or study, because of the fear: see your doctor this week.
  • A SPIN score that stays high or keeps rising after the quest, or fear with a big impact on your life: see your doctor.
  • Drinking or using drugs to get through every social event: tell your doctor. It does not rule out treatment, and heavy drinking gets its own help.
  • Low, flat and uninterested for weeks as well: tell your doctor, and say which came first. See Low mood.
  • Sudden attacks of racing heart, sweating and fear, out of the blue as well as in company: ask about panic disorder.
  • Worry about one part of how you look, mirror checking and hiding it, behind the avoidance: ask about body dysmorphic disorder.
  • Nervous, shaky, losing weight and always hot: ask for a thyroid blood test.
  • Anxiety or paranoia that started with cannabis, or on or after anabolic steroids: tell the doctor.
  • Suspicion that people are against you, hearing voices, or lifelong avoidance for fear of rejection: the doctor checks for psychosis and avoidant personality disorder.
  • Your doctor will also check for autism and other anxiety or mood problems that can look like social anxiety.
