Buffs

- A single brief conversation lowers weekly intake a year later
- Longer counseling adds nothing over brief advice
- Cutting intake lowers blood pressure above two drinks a day
- Cutting intake cuts atrial fibrillation recurrence and burden
- Smaller servings cut how much is drunk in a session
- Alcohol delays and shortens REM sleep from about two drinks
- Digital tools lower weekly intake with no clinician involved
- A month off leaves intake lower months afterwards
- One month off improves insulin resistance, weight and blood pressure
- If-then plans lower weekly intake
- Dropping a risk drinking level tracks fewer consequences and better mood
- Removing the largest serving from the range lowers what a room drinks
- Self-monitoring blood alcohol lowers drinking in heavy drinkers
- Dropping two risk drinking levels tracks lower cardiovascular disease
- Doing the dry month alongside other people raises completion

Debuffs

- **Know first**Physical dependence, where cutting abruptly is a medical event and not a plan
- **Know first**Morning drinking, blackouts or a past withdrawal seizure, which put a man outside this record
- **Manageable**Substitution, where the beer comes down and the wine goes up
- **Manageable**If-then rules losing their effect over the months after they are written
- **Manageable**Counting nudging intake upward in men who were already drinking lightly
- **Manageable**A dry month followed by the old number returning if nothing else changed
- **Mild**Social friction in the first weeks, which is where participants asked for the most support
- **Mild**Living with other drinkers making a dry month measurably harder to finish

“Take the smaller glass every round. You don't have to quit to collect the benefit.”

## Start today

1. Order the smaller glass tonight, and keep ordering it every round for a month.

## Why it works

Four moves, each tested on its own, for the man who is not dependent and is drinking more than he meant to. A short conversation that ends with a written number. Counting what actually goes in, in grams. A rule set down while sober about what happens in the situation that always gets him. And one month off to reset the baseline. None of them ask him to stop for good, and the data says he does not have to: the health gains arrive on a slope, and he starts collecting them at the first level down.

Drinking four or five nights a week is not a decision made forty times a month. It is a default that runs on cues: the hour, the room, the people, the glass put in front of you. Counting turns an unmeasured habit into a measured one. A short conversation puts a number on it and forces a target. An if-then rule moves the decision out of the moment you are already drinking and into a moment when you are not. A dry month breaks the cue chain outright. It rebuilds the belief that you can be in the room and say no. Serving size sets the unit the whole habit is counted in. So it changes the total without changing a single choice.

Measuring a habit changes it, and the direction depends on where you started. Across 787,393 breath alcohol readings from 32,179 people who got a personal breathalyser, heavy drinkers' readings fell over time. They went from an average of 0.106 percent to 0.096 percent. Their odds of underestimating where they were fell with it. The device did nothing but show a number the man had already guessed at. For heavy drinkers the guess was low, and the gap between the guess and the reading closed as use went on.

Men drink servings, not grams. Shrink the serving and the count stays the same while the total falls. Cutting the standard serving by a quarter cut what people actually drank in an hour by 20.7 to 22.3 percent. Cutting it by roughly a third in a real bar cut three-hour drinking by 32.4 to 39.6 percent. The glass is the unit, and the unit can change.

It does not work by detox. It works by building drink-refusal confidence: the belief that you can be in the room and not drink. Across 857 British adults doing Dry January, everyone who took part was drinking less at six months. They were also more confident they could turn a drink down, whether or not they finished the month. The change was bigger in those who finished. Almost nobody bounced back above where they started. A month is long enough to break the autopilot and short enough that a man will actually agree to it.

The benefit of cutting is not a step at zero. It is a slope. Blood pressure responds only above about two drinks a day, and then it responds hard. Men drinking six or more a day who halved their intake dropped systolic pressure by 5.50 mmHg and diastolic by 3.97\. In 1,383 patients tracked through a treatment trial, any drop in World Health Organization risk drinking level predicted fewer alcohol-related problems and better mental health for up to a year. Quitting was not needed. A bigger drop predicted a bigger improvement. Cutting down is the outcome, not a consolation prize for failing to quit.

## What it costs you

An hour of attention, and the willingness to write a number down. The honest frame is that each of these moves is worth a few drinks a week and they stack, rather than any one of them being the answer. Two things are worth knowing before starting. If-then rules decay over the months after they are written, so they get rewritten rather than remembered. And measurement is not neutral for everyone: in the breathalyser data, light drinkers who started tracking drifted up rather than down, from a mean 0.058 percent to 0.067 percent.

## Questions

How do I know if I am actually drinking too much?

Three questions, two minutes. How often you have a drink. How many on a typical drinking day. How often you have six or more on one occasion. That is the AUDIT-C. In male patients it picked out heavy drinkers with an accuracy score of 0.891 out of 1, beating the full ten-item version at the same job. In 103,567 adults followed for 879,964 person-years, men's risk of an alcohol-related medical event rose 45 percent per point. The score that best predicted future harm in men aged 20 to 64 was six.

Do I have to quit?

No, and the data is unusually clear on this. Blood pressure responds to a cut, not to abstinence, and only above two drinks a day: at six or more a day, halving intake took 5.50 mmHg off systolic pressure. Even the atrial fibrillation trial, framed as abstinence, ran at 2.1 drinks a week rather than zero. Aim at the next level down and collect the benefit there.

What is the one thing to do this week?

Count, in grams, and use the smaller glass. Counting is the easiest move and gives the fastest feedback. In the breathalyser data, heavy drinkers' readings fell over time, from an average 0.106 to 0.096 percent, and their odds of underestimating fell with it. The glass is the other half. A quarter off the serving takes a fifth off the session. A third off takes a third off. You make no extra decision.

Is Dry January theater?

No. It shows up in the numbers. In 1,886 French adults tracked from December to October the next year, AUDIT-C scores fell by a medium margin at three months and were still down by a medium margin at nine. The biggest falls were in the heaviest drinkers. In 857 British people, both those who finished and those who did not were drinking less six months later. Almost nobody came back drinking more. In the body, one month at zero improved insulin resistance by 25.9 percent and systolic pressure by 6.6 percent. It moved two cancer-related growth factors by 41.8 and 73.9 percent.

Does an app do it, or do I need to see someone?

An app does it. A review of 80 trials found digital tools cut 4.12 g of alcohol a day in the 66 trials that measured it. They took 1.11 heavy drinking sessions off the month in 26, and dropped AUDIT scores 3.04 points in 9\. The largest single UK trial took in 5,602 higher-risk drinkers. At six months, the Drink Less app came in 1 US standard drink (2.00 UK units) a week further down than the NHS advice page.

I plan on two and finish on six. What fixes that?

A rule written while sober that names the exact situation and the exact response. Not a target. Across 16 studies these plans cut weekly drinking. In two trials run in heavy drinkers, they took off about 4 and 3 US standard drinks (6.9 and 5.7 units) a week within a month. Write it on paper: paper gave a small effect, online a tiny one. Rewrite it now and then, because the effect fades. And set the serving before the evening starts. That is the part of the night your later self does not get a vote on.

Will drinking less actually fix my sleep?

Yes, and earlier than you would expect. Across 27 studies, REM sleep is pushed back and cut short from about two standard drinks, and it degrades further with every drink after. The sedation men are chasing, falling asleep faster, only appears at around five drinks, and that is precisely the dose that guts the rest of the night.

Does anyone else need to be involved?

It helps, and it shows. In 634 adults starting a one-month alcohol-free challenge, four things predicted finishing. Confidence in turning down a drink. Planning to keep drinking less afterwards. Not living with other drinkers. And doing it alongside someone else. What people wanted more help with was the social side. Recruit one other person and decide in advance what you say.
